Cardiac Therapy

Uses

Cardiogenic shock in infarction or cardiac surgery.

How to take it

Adults: Continuous IV infusion. 1-5 mcg/kg/minute, up to 20 mcg/kg/minute, max 50 mcg/kg/minute. Doses above 20 mcg/kg/minute may not have beneficial effect on blood pressure and may increase the risk of tachyarrhythmias. Infusion may be increased by 1-4 mcg/kg/minute at 10-30 minute interval until desired result. Children: Not licensed for use in children under 12 yr. To correct the haemodynamic imbalance due to acute hypotension, shock, cardiac failure, adjunct following cardiac surgery: Continuous IV infusion. 12-18 yr. initially 5 mcg/kg/minute adjusted according to response (max 20 mcg/kg/minute). Side effects

Nausea and vomiting, peripheral vasoconstriction, hypotension, hypertension, tachycardia.

Pregnancy and breastfeeding

Pregnancy: Risk cannot be ruled out. Lactation: Caution advised or effect undetermined. Contraindications: Tachyarrhythmia, pheochromocytoma. Precautions: Correct hypovolaemia; low dose in shock due to acute myocardial infarction; hyperthyroidism.

Advice

Contact prescriber immediately if there is chest pain, palpitations, rapid heartbeat, headache, nervousness, restlessness, nausea, vomiting or respiratory difficulty. Regular monitoring of haemodynamic parameters, such as central venous pressure, pulmonary artery wedge pressure, and cardiac output is necessary. Urine output is monitored hourly to check renal blood flow.

MAO inhibitors, alpha- and beta-adrenergic blockers, cocaine, general anaesthesia, methyldopa, phenytoin, reserpine, TCAs, guanethidine.
