Dopamine

Cardiac Therapy · Beta-1 agonist

Indications

Cardiogenic shock in infarction or cardiac surgery.

Dosage

Adults: Continuous IV infusion. 1-5 mcg/kg/minute, up to 20 mcg/kg/minute, max 50 mcg/kg/minute. Doses above 20 mcg/kg/minute may not have beneficial effect on blood pressure and may increase the risk of tachyarrhythmias. Infusion may be increased by 1-4 mcg/kg/minute at 10-30 minute interval until desired result. Children: Not licensed for use in children under 12 yr. To correct the haemodynamic imbalance due to acute hypotension, shock, cardiac failure, adjunct following cardiac surgery: Continuous IV infusion. 12-18 yr. initially 5 mcg/kg/minute adjusted according to response (max 20 mcg/kg/minute).

Administration

Administer by IV infusion into a large vein using an infusion pump or other infusion control device. Pre-mixed infusion solution are suitable for administration without dilution, but the concentrated injection must be diluted for use. The dose of concentrate is added to 250 or 500 mL of compatible solution. Typical dilution is 400 mg in 250 mL to yield 1.6 mg/mL. Do not exceed 3.2 mg/mL. Compatibility: Stable in D5LR, D5 ½NS, D5NS, D5W, D10W, LR, mannitol 20%, NS. Incompatible with sodium bicarbonate 5 % and alkaline solutions or iron salts. At Y-site administration, incompatible with acyclovir, alteplase, amphotericin B cholesteryl sulphate complex, cefepime, indomethacin, insulin (regular), thiopental. When admixed, incompatible with acyclovir, alteplase, amphotericin B, ampicillin, metronidazole with sodium bicarbonate, penicillin G potassium. Stability: Store vials at controlled…

Nausea and vomiting, peripheral vasoconstriction, hypotension, hypertension, tachycardia.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Caution advised or effect undetermined. Contraindications: Tachyarrhythmia, pheochromocytoma. Precautions: Correct hypovolaemia; low dose in shock due to acute myocardial infarction; hyperthyroidism.

Interactions

MAO inhibitors, alpha- and beta-adrenergic blockers, cocaine, general anaesthesia, methyldopa, phenytoin, reserpine, TCAs, guanethidine.

Advice to Patient

Contact prescriber immediately if there is chest pain, palpitations, rapid heartbeat, headache, nervousness, restlessness, nausea, vomiting or respiratory difficulty. Regular monitoring of haemodynamic parameters, such as central venous pressure, pulmonary artery wedge pressure, and cardiac output is necessary. Urine output is monitored hourly to check renal blood flow.
