About Sodium Clodronate
Drugs affecting Bone Metabolism
Uses
Osteolytic lesions, hypercalcaemia and bone pain associated with skeletal metastasis in breast cancer or multiple myeloma.
How to take it
ORAL: Adults: 1.6 gm once daily in the morning; max 3.2 gm daily in two divided doses. Doses should be taken =2 hr after and =1 hr before food. Children: Not recommended. INJECTION: Adults: Initially 300 mg daily by slow IV infusion for up to 7 days. Maintenance use capsules. Children: Not recommended. Continuous in Glucose 5% or NaCl 0.9%. Dilute 300 mg in 500 mL & give over at least 2 hours or 1.5 gm in 500 mL & give over at least 4 hours.
Side effects
GI upset, renal dysfunction, asymptomatic hypocalcaemia. Transient proteinuria. Reversible elevations of parathyroid hormone, creatinine, lactate dehydrogenase, transaminase, alkaline phosphatase. Rarely: Skin reactions.
Pregnancy and breastfeeding
Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Severe renal impairment (CrCl <10 mL/min). Precautions: Moderate renal impairment (CrCl 10-49 mL/min). Monitor renal and hepatic function and white cell counts. Monitor serum calcium and phosphate periodically. Risk of rare osteonecrosis of the jaw usually associated with dental procedures or with concomitant risk factors such as cancer, chemotherapy, radiotherapy or corticosteroid treatment, smoking, history of dental disease and poor oral hygiene.
Advice
Maintain adequate fluid intake. Avoid invasive dental procedures and advise reporting of any oral symptoms while on treatment, consider dental examination prior to treatment.
