About Atenolol
Betablocking Agent
Uses
Management of hypertension, angina pectoris. Cardiac dysrhythmias. Treatment of myocardial infarction. Early and late intervention after myocardial infarction.
How to take it
Adults: ORAL: Hypertension/Arrhythmias/Early intervention in acute MI: 50 or 100 mg once daily. Angina: Usually 100 mg once daily or 50 mg twice daily. Adults: INJECTION: Arrhythmias: IV injection: 2.5 mg at a rate of 1 mg/min, repeated at 5-min intervals to a max 10 mg. Excessive bradycardia can be countered with IV injection of atropine sulphate 0.6-2.4 mg in divided doses of 600 mcg. IV Infusion: 150 mcg/kg over 20 min, repeated every 12 hr if necessary. Early intervention within 12 hr of myocardial infarction: By IV injection: 5 mg over 5 min, then orally 50 mg after 15 min, 50 mg after 12 hr, then 100 mg daily. Side effects
Cardiovascular effects including bradycardia, postural hypotension. Rarely, heart failure, increased AV block, cardiac arrhythmias. Raynaud's phenomenon cold extremities, sexual dysfunction. GI, sleep and CNS disturbances. Dyspnoea rarely, bronchospasm. Skin reactions, withdraw if unexplained dry eyes or rash. Fatigue, paraesthesia, peripheral neuropathy, arthralgia, myopathy, altered blood sugar, haematology and lipid levels.
Pregnancy and breastfeeding
Pregnancy: Positive evidence of risk: Use only when no safer alternative exists for a serious problem. Lactation: Caution advised or effect undetermined. Contraindications: 2nd or 3rd degree AV block (without pacemaker), severe bradycardia, sick sinus syndrome, uncontrolled heart failure, cardiogenic shock, severe peripheral arterial disease, hypotension, metabolic acidosis, untreated pheochromocytoma, severe asthma or COPD, patients at risk of hypoglycaemia, Prinzmetal's angina. Precautions: 1st degree AV block. Heart failure (avoid if uncontrolled). Peripheral arterial circulatory disorders (avoid if severe), Myasthaenia gravis. Ischaemic heart disease, withdraw gradually. Advice
Take with or without meals. Take a missed dose as soon as possible. However, if it is within 8 hours of the next scheduled dose, skip the missed dose and follow regular schedule. Contact prescriber if there is unrelieved swelling of extremities, difficulty in respiration, cough, unresolved fatigue, unusual weight gain, unresolved constipation or unusual muscle weakness. Check blood pressure and pulse regularly before taking medication. If you have diabetes, regularly check blood glucose level. Atenolol masks hypoglycemia. Change position slowly.

Interactions with Atenolol
Ca++ antagonists. Antihypertensives including centrally acting agents (e.g. clonidine), antiarrhythmics, antipsychotics. TCADs, MAOIs, fluvoxamine, barbiturates, cardiac glycosides, cimetidine, hydralazine, rifampicin, insulin/oral hypoglycaemics, NSAIDs, ergot derivatives, erythromycin, anaesthetics, diuretics, digoxin, xanthines, antimalarials, moxisylyte, iodinated contrast media, baclofen, levothyroxine, alcohol, corticosteroids, rizatriptan, muscle relaxants, contraceptives, parasympathomimetics, fingolimod, ampicillin.
