About Nilotinib
Cytostatic
Uses
Treatment of chronic myeloid leukaemia.
How to take it
Adults: 400 mg every 12 hr (800 mg/day). Children: 230 mg/m2 every 12 hr, rounded to the nearest 50 mg dose. Max: 400 mg/dose every 12 hr.
Side effects
Flushing, skin papilloma, peripheral arterial stenosis, pyrexia, insomnia, depression, anxiety, angina, arrhythmia, eye disorders, pancreatitis, pollakiuria, increased creatine kinase, amylase and insulin, bilirubin or lipase; reduce dose or interrupt treatment, skin disorders, fatigue, headache, GI upset, alopecia, myalgia, arthralgia, muscle spasms, bone pain, pain in extremity, asthenia, oedema, anorexia, dysgeusia, dysphonia, epistaxis, blood dyscrasias. Wt change, palpitations, QT prolongation, dizziness, paraesthesia, hypoaesthesia, malaise, hepatic dysfunction, peripheral neuropathy, pain, folliculitis, upper RTI, dyspnoea, cough, night sweats, electrolyte disturbances, hypertension.…
Pregnancy and breastfeeding
Pregnancy: Risk cannot be ruled out. Lactation: Caution advised or effect undetermined. Precautions: Do not use in patients with hypokalaemia, hypomagnesaemia or long QT syndrome. Myelosuppression, hepatic impairment, uncontrolled or significant cardiac disease, history of pancreatitis. Patients with total gastrectomy. Tumour lysis syndrome. Galactose intolerance, severe lactase deficiency, glucose-galactose malabsorption. Monitor QTc, CBC. Test for HBV before starting treatment and consult specialist if test positive; monitor for reactivation during and for several months after stopping treatment. Advice
Take atleast 2 hours before or 1 hour after meals. Do not chew or crush capsules, swallow whole with a glass of water. Report immediately if there is severe nausea or vomiting with stomach pain, chest pain, palpitations, difficulty in walking or speaking, leg pain, numbness, weakness, unusual bruising or bleeding, blood in urine or stool, yellowing of eyes or skin, sudden headache, change in vision, unconsciousness. Do not take grapefruit juice and products containing grapefruit during therapy. If you cannot swallow capsules, open the capsule and sprinkle contents into a teaspoon of applesauce. Interactions with Nilotinib
Strong CYP3A4 inhibitors (e.g. ketoconazole, itraconazole, voriconazole, ritonavir, clarithromycin, telithromycin), CYP3A4 inducers (e.g. phenytoin, rifampicin, carbamazepine, phenobarbitone, St. John's Wort), CYP2C8, CYP2C9 and CYP2D6 substrates, antiarrhythmics (e.g. amiodarone, disopyramide, procainamide, quinidine, sotalol), drugs that prolong QT interval (e.g. chloroquine, halofantrine, clarithromycin, haloperidol, methadone, moxifloxacin, bepridil, pimozide). Grapefruit juice and foods. Midazolam, ergot alkaloids, warfarin, antiarrhythmics or other drugs that prolong QT interval, H2 antagonists, antacids.
