Tofacitinib

Anti-rheumatic, Systemic · Janus associated kinase inhibitor

Indications

Rheumatoid arthritis.

Dosage

Adults: 5 mg twice daily. Children: Not recommended.

Adverse Effects

Hypertension, headache, diarrhoea, UTI, increase ALT, increase creatinine, upper RTI, nasopharyngitis, serious infection, abdominal pain, anaemia, dehydration, drug induce liver injury, dyspepsia, dyspnoea, erythema, gastritis, hepatic steatosis, lymphocytopenia, musculoskeletal pain, neutropenia, peripheral edema, pruritus, pyrexia, rash, sinus congestion, tendonitis, tuberculosis, vomiting.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Severe hepatic impairment, live vaccination, tuberculosis, serious infections, lymphoma. Precautions: Patients who have been exposed to tuberculosis, with history of serious or opportunistic infection, patients who live in or travel to/from areas of endemic mycoses (blastomycosis, coccidioidomycosis, histoplasmosis). Elderly patients. Patients at increased risk for gastrointestinal perforation (e.g. history of diverticulitis).

Interactions

BCG, CYP3A4 inducers, natalizumab, tacrolimus, leflunomide, fluconazole, denosumab, dasatinib, ivacaftor, sitaxsentan, mifepristone, roflumilast, pimecrolimus.

Advice to Patient

Take with or without meals. Contact prescriber if there is severe tiredness, yellowing of skin or eyes, loss of appetite, fever, chills, vomiting, dark coloured stool, abdominal upset, muscle pain, dark urine, skin rash.

Absorption: Conventional tab: Rapidly and well absorbed. Bioavailability: 74%. Metabolism: Hepatic. Half-life elimination: Conventional tab, oral soln: Approx. 3 hrs. Extended-release tab: Approx. 6-8 hrs. Time to peak, plasma: Conventional tab, oral soln: 0.5-1 hr. Extended-release tab: 4 hrs. Excretion: Mainly via urine (30%)
