Tigecycline
Tetracycline · Glycylcycline antibiotic structurally related to tetracycline
Indications
Complicated intra-abdominal infections, skin and soft tissue infections.
Dosage
Adults: By IV infusion, initially 100 mg then 50 mg every 12 hr for 5-14 days. Children: Not recommended.
Administration
Administered by IV infusion over 30 to 60 min after dilution of the reconstituted solution in 100 mL of Dextrose 5% or NaCl 0.9 %. Reconstitution: Reconstitute the vials with 5.3 mL of NaCl 0.9% or D5W. The concentration should not exceed 1 mg/mL. The solution should be yellow to orange. Compatibility: Dextrose 5%, NaCl 0.9%. Stability: Reconstituted vial at room temperature for upto 6 hours with an additional 18 hours after dilution in the infusion solution or 48 hours in refrigerator.
Adverse Effects
Gl disturbances, anorexia, dizziness, phlebitis, rash, headache, abscess, infections, raised ALT, AST, BUN, amylase, prolonged APTT/PT, raised INR, pruritus, inj site reactions, impaired healing, hypoglycaemia, hypoproteinaemia, increased LFTs, hepatic failure, pseudomembranous colitis and anaphylaxis.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Foot infections in diabetic patients. Precautions: Severe hepatic impairment, cholestasis, monitor for signs of acute pancreatitis, consider withdrawal if suspected. Tetracycline hypersensitivity. Severe underlying disease or concurrent bacteremia. Monitor for superinfection. Children: Monitor LFTs, coagulation, haematology, amylase and lipase.
Interactions
Anticoagulants, P-gp inhibitors or inducers.
Advice to Patient
Contact prescriber if there is itching, rash, severe diarrhoea, watery or bloody stools, pain at infusion site. Watery or bloody stools can occur 2 or more months after therapy and can be serious, requiring immediate treatment.
Pharmacokinetics
Metabolism: Hepatic. Half-life elimination: Single dose: 27 hrs; following multiple doses: 42 hrs; increased by 23% in moderate hepatic impairment and 43% in severe hepatic impairment. Excretion: Faeces (59%). Urine (33%).
