Antiparkinsonism Preps · MAO-B inhibitor
Indications
Parkinsonism in conjunction with levodopa therapy particularly in patients on levodopa who develop on-off symptoms/ other dyskinesias.
Dosage
Adults: 2 tab as a single dose in the morning or in divided doses at breakfast and lunch. Children: Not recommended.
Adverse Effects
Postural hypotension, BP changes, involuntary movements (reduce levodopa dose). Nausea, confusion or psychosis, bradycardia, chest pain, GI upset, headache, tremor, dizziness, depression, hallucinations, agitation, dry mouth, sore throat, nasal congestion, difficulty in micturition, skin reactions, joint or muscle pain, muscle cramps, increased dyskinesias, insomnia, abnormal dreams, transient increase in liver enzymes, hyperhidrosis, falls.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Active duodenal or gastric ulcer. Precautions: Severe renal or hepatic impairment, avoid abrupt withdrawal, uncontrolled hypertension, arrhythmias, psychosis, acute porphyria, history of peptic ulcer. Surgery.
Interactions
MAOIs, alcohol, antidepressants, opioids, dopamine, sympathomimetics, serotonin agonists, CNS stimulants or depressants, oral contraceptives, HRT, amantadine, anticholinergics, digoxin, anticoagulants, drugs that affect BP.
Advice to Patient
Take with meals. Take at the same time each day. Avoid taking in late afternoon and evening. Take disintegrating tablets before breakfast without any liquid. Wash hands well after application of transdermal patch and dispose off removed patch immediately. Rotate sites for application of patch. If a patch falls off, apply a new patch to a new site and to return to previous schedule. Contact prescriber if there is acute headache, unrelieved vomiting or constipation, loss of memory, chest pain, palpitations, irregular heartbeat, nervousness, seizures, hallucinations, skin rash, increased muscle spasm, rigidity, involuntary movements, painful or difficult urination, dry mouth that lasts more than 2 weeks, increased urge for any activity or if there is worsening of condition. Pharmacokinetics
Absorption: Cap/tab: Bioavailability increases 3- to 4-fold when taken with food. Orally disintegrating tab: Rapid; greater bioavailability than cap/tab. Food decreases Cmax and AUC to approx. 60% of fasting state. Transdermal: 25%-30% (of total selegiline content) over 24 hrs. Metabolism: Hepatic. Half-life elimination: Oral: 10 hrs. Excretion: Urine (primarily metabolites). Faeces.
