Secukinumab

Cytostatic · Monoclonal antibody

Indications

Moderate to severe plaque psoriasis.

Dosage

Adults: 300 mg by SC inj, repeated once weekly at weeks 1, 2 and 3 then monthly starting at week 4. Consider discontinuation if no response after 16 weeks. Children: Not recommended.

Administration

Administer via SC route. Reconstitution: Reconstitute with SWFI. The resulting solution should be clear and colourless to slightly yellow. Stability: Store in a refrigerator (2°C-8°C). Do not freeze. Protect from light.

Adverse Effects

Upper RTI, diarrhoea, rhinorrhoea, oral herpes.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Clinically important, active infection, including TB. Precautions: Crohn’s disease. Renal or hepatic impairment. Latent TB; consider treatment before starting secukinumab. Latex sensitivity. Chronic infection or history of recurrent infection. Advise patients to report any signs of infection; discontinue if severe infection occurs.

Interactions

Live vaccines, CYP3A4 substrates, other immunosuppressants.

Pharmacokinetics

Bioavailability: 55-77%. Metabolism: Expected to be degraded into small peptides and amino acids via catabolic pathways. Half-life elimination: 22-31 days. Time to peak, plasma: Approx. 6 days.
