Muscle Relaxant · Non-depolarising muscle relaxant
Indications
Adjunct to general anaesthesia to facilitate tracheal intubation during routine and rapid sequence induction and to provide muscle relaxation for procedures.
Dosage
Adults: Intubation: By IV injection, initially 600 mcg/kg, maintenance by IV inj, 150 mcg/kg (Elderly: 75-100 mcg/kg) or maintenance by IV infusion, 300-600 mcg/kg/hr (Elderly: up to 400 mcg/kg/hr) adjusted according to response. Intensive care: By IV inj, initially 600 mcg/kg (optional), maintenance by IV infusion, 300-400 mcg/kg/hr for first hr, then adjusted according to response. Children: Not licensed for use in children for assisted ventilation in intensive care. To avoid excessive dosage in obese children, dose should be calculated on the basis of ideal weight. Administration
Administer IV only, may be given undiluted as a bolus injection or via a continuous infusion using an infusion pump. Compatibility: Stable in 0.9% NaCl and D5W, this mixture should be used in 24 hours of preparation. Stability: Store under refrigeration (2°C-8°C) Do not freeze, when stored at room temperature it is stable for 30 days.
Adverse Effects
High dosage produce mild vagolytic activity. Rash; injection site reactions.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Risk cannot be ruled out. Lactation: Caution advised or effect undetermined. Precautions: Prior anaphylactic reactions; neuromuscular disease; acid-base, electrolyte imbalance; dehydration; hypothermia; obesity; cardiovascular disease; oedema; renal, hepatic, pulmonary impairment; elderly, infants (1-12 months).
Advice to Patient
Continuous monitoring is required during and after administration. After muscle tone becomes normal, do not change position and rise up without help. Contact prescriber if there is difficulty in breathing, muscle tremors, fast heartbeat, skin rash, hives.
Pharmacokinetics
Onset of action: Infants = months and children: 30 seconds to 1 minute. Adults: Good intubation conditions within 1 to 2 minutes (depending on dose administered); maximum neuromuscular blockade within 4 minutes. Duration: Infants: 3 to 12 months: 40 minutes. Children: 1 to 12 years: 26 to 30 minutes. Adults: Approx. 30 minutes (with standard doses, increases with higher doses and inhalational anesthetic agents). Metabolism: Minimally hepatic. Half-life elimination: Alpha elimination: 1 to 2 minutes. Beta elimination: Infants 3 to 12 months: 1.3 + 0.5 hours. Children 1 to <3 years: 1.1 + 0.7 hours. Children 3 to <8 years: 0.8 + 0.3 hours. Adults: 1.4 to 2.4 hours. Hepatic impairment: 4.3 hours. Renal impairment 2.4 hours. Excretion: Feces (31%); urine (26%).
