Indications

Cardiac arrhythmias.

Dosage

Ventricular arrhythmias: Adults: Max 400 mg every 6 hr (immediate release) in severe refractory ventricular tachycardia. Body weight < 50 kg: Immediate release, initial loading dose 200 mg if rapid onset required, maintenance 100 mg every 6 hr. Controlled release, maintenance dose 200 mg every 12 hr. Body weight > 50 kg: Immediate release, initial loading dose 300 mg if rapid onset required, maintenance 150 mg every 6 hr and if no response seen within 6 hr of loading dose, may increase maintenance dose to 200 mg every 6 hr. Controlled release, maintenance dose 300 mg every 12 hr. Children: Not recommended.

Adverse Effects

Anticholinergic effects. Very rarely psychosis, jaundice, hypoglycaemia.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: 2nd or 3rd degree AV block, sinus node disease in absence of pacemaker. Cardiogenic shock. Severe uncompensated cardiac failure. Precautions: 1st degree AV block. Cardiac failure should be controlled with glycosides. Renal or hepatic failure. Prostatic hypertrophy, glaucoma, urinary retention, hypokalaemia.

Interactions

Other antiarrhythmics, beta-blockers, diuretics, anticholinergics.

Advice to Patient

Do not stop taking medication suddenly. Take a missed dose as soon as you remember, unless it is nearly time for the next dose. Do not double the dose. Inform prescriber if more than one dose is missed. Check pulse rate regularly and report important changes. Change position slowly from lying or sitting position to reduce dizziness. Avoid too much heat and exercise.

Pharmacokinetics

Absorption: Readily and almost completely absorbed from the GI tract. Metabolism: Partially metabolized in the liver. Half-life elimination: 4-10 hrs. Increased in hepatic and renal impairment and in heart failure. Time to peak, plasma: 0.5-3 hrs after oral dosing. Excretion: Urine (majorly). Faeces (10%).
