Antihistamine, Systemic · Piperidine derivative

Indications

Treatment of acute & chronic allergies & pruritus. Therapy for anaphylactic reactions, adjunctive to epinephrine & other standard measures after the acute manifestation have been controlled.

Dosage

Adults: Initially 4 mg three times daily, max: 32 mg/day. Children: 7-14 yr: 4 mg two-three times daily, max: 16 mg/day; 2-6 yr: Initially 2 mg two-three times daily, max: 12 mg/day.

Adverse Effects

Sedation, transient sleepiness, dizziness, disturbed coordination, confusion, restlessness, excitation, nervousness, tremor, irritability, insomnia, paraesthesia, neuritis, convulsions, euphoria, hallucinations, hysteria, faintness, rash & oedema, excessive perspiration, urticaria, photosensitivity, acute labyrinthitis, blurred vision, diplopia, vertigo, tinnitus, hypotension, palpitation, tachycardia, extrasystoles, anaphylactic shock, haemolytic anaemia, leukopenia, agranulocytosis, thrombocytopenia, dryness of mouth, epigastric distress, anorexia, nausea, vomiting, diarrhoea, constipation, jaundice, difficult micturition, urinary retention, early menses, dry nose & throat, thickening of bronchial secretions, tightness of chest, wheezing, nasal stuffiness, fatigue, chills, headache.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Acute asthma attack. Hypersensitivity. Angle-closure glaucoma. Stenosing peptic ulcer. Symptomatic prostatic hypertrophy. Bladder neck obstruction. Pyloroduodenal obstruction. Elderly, debilitated patients. Children below 2 years age. Precautions: History of bronchial asthma. Increased intraocular pressure. Hyperthyroidism. CV disease. Hypertension. May impair ability to drive or operate machinery.

MAOIs, CNS depressants.

Advice to Patient

Take before meals. Contact prescriber if there is constant agitation, confusion, increased sleepiness, blurred vision, urinary pattern changes, sore throat, chest pain, palpitations, difficulty in breathing, expectoration or if there is no improvement or worsening of condition. Do not take other depressants, sleep-inducing medications, alcohol without consulting prescriber.

Pharmacokinetics

Absorption: Well absorbed. Metabolism: Primarily hepatic glucuronidation. Half-life elimination: Metabolites: Approx. 16 hours. Time to peak, plasma: 6 to 9 hours. Excretion: Urine (approx. 40% primarily as metabolites); feces (2% to 20%, <6% as unchanged drug).
