About Finerenone

Diuretic

Uses

Used to reduce the risk of sustained eGFR decline, end stage kidney disease, cardiovascular death, non-fatal MI, and hospitalization for heart failure in adult patients with chronic kidney disease (CKD) associated with type 2 diabetes.

How to take it

Adults: Initially 10 mg or 20 mg orally once daily based on eGFR and serum potassium thresholds. Increase dosage after 4 wks to the target dose of 20 mg once daily, based on eGFR and serum potassium thresholds. Children: Not recommended.

Side effects

Hypotension, decreased eGFR, hyperkalaemia, hyponatraemia, pruritus.

Pregnancy and breastfeeding

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Severe renal impairment (eGFR <25ml/min/1.73m2), severe hepatic impairment, serum potassium >5mmol/L. Addison's disease. Precautions: Moderate hepatic impairment. Heart failure with reduced ejection fraction (NYHA class II-IV). Measure serum potassium and eGFR at baseline and 4 weeks after starting treatment or increasing dose; ensure additional serum potassium monitoring for first 4 weeks if 4.9-5mmol/L at baseline. Discontinue if eGFR declines to <15ml/min/1.73m2 and withhold if serum potassium increases to >5.5mmol/L.

Advice

Take with or without meals. Take missed dose as soon as possible, but on same day only. If this is not possible, skip the dose and continue with the next dose as prescribed. Consult prescriber before using potassium supplements or salt substitutes containing potassium. Avoid grapefruit and grapefruit juice during therapy.

Interactions with Finerenone

CYP3A4 inhibitors (e.g, azole antifungals, ritonavir, macrolides, cobicistat) or inducers (eg, phenytoin, rifampicin, phenobarbital, carbamazepine, St John's wort), potassium-sparing diuretics, other aldosterone antagonists, potassium suppl, trimethoprim, antihypertensives, grapefruit juice.
