Hypoglycaemic excluding Insulin

Uses

Monotherapy for type II diabetes, particularly overweight patients, inadequately controlled by diet and exercise for whom metformin is inappropriate. As dual therapy in combination with metformin particularly in overweight patients, or in combination with a sulphonylurea in patients with an intolerance or contraindication to metformin, for type II diabetes inadequately controlled by maximal tolerated doses of either metformin or a sulphonylurea. In combination with metformin and a sulphonylurea, particularly overweight patients, for type II diabetes inadequately controlled by dual therapy.

How to take it

Adults: Initial: 2 mg twice daily or 4 mg daily. Titrate: May increase after 8-12 wk to 4 mg twice daily or 8 mg daily. Max: 8 mg/day as monotherapy or with metformin, sulfonylureas, or sulfonylureas plus metformin; 4 mg/day with insulin. Combination Therapy With Insulin: Decrease insulin by 10-25% if hypoglycaemic or FPG <100 mg/dL; individualize further adjustments based on glucose lowering response. Children: Not recommended.

Side effects

Anaemia, hypoglycaemia, weight gain, headache, GI upset, fatigue. Oedema, hypercholesterolaemia.

Pregnancy and breastfeeding

Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Hepatic impairment, raised liver enzymes, cardiac failure or history of cardiac failure. Precautions: Severe renal impairment. Monitor for signs of fluid retention, including weight gain, or other signs of cardiac failure (discontinue if deteriorates). Monitor for signs of hepatic dysfunction and measure liver enzymes before treatment and periodically thereafter. Monitor for weight gain and anaemia.

Advice

Take with meals. Do not skip meals. If a dose is missed at the usual meal timings, take it with next meal. If daily dose is missed completely, do not double dose. Contact prescriber if there is nausea, vomiting, abdominal pain, shortness of breath, chest pain, fatigue, anorexia, rapid heartbeat, swelling of ankles, leg or abdomen, rapid weight gain, fever, rash, yellowing of skin or eyes or dark urine. If these are present, medication may need to be discontinued. Avoid alcohol. Wear or carry identification card at all times indicating diabetes. Keep candy or other source of sugar to treat mild hypoglycaemia. Avoid direct sunlight, use sunscreen and wear protective clothing when outside to…

Interactions with Rosiglitazone

Insulin (contraindicated), NSAIDs, paclitaxel, gemfibrozil, rifampicin, other inducers of CYP2C8.
