Uses
Symptomatic treatment of mild to moderately severe Alzheimer's dementia or in Parkinson's disease.
How to take it
ORAL: Adults: Initially 1.5 mg morning and evening with meals. If well tolerated, increase dose by 1.5 mg at minimum 2 wk intervals first to 3 mg twice daily, then to 4.5 mg twice daily and finally 6 mg twice daily. Maintain patient on highest tolerated dose; up to max 6 mg twice daily. PATCHES: Adults: Initially apply 4.6 mg/24 hr to clean, dry, non-hairy, non-irritated skin on back, upper arm, or chest, removing after 24 hr and sitting a replacement patch on a different area (avoid using the same area for 14 days); if well tolerated increase to 9.5 mg/24 hr patch daily after at least 4 wk for further 6 mths, then increased if necessary to 13.3 mg/24 hrs daily, use caution in patients with…
Side effects
Asthaenia, anorexia, dizziness, somnolence, abdominal pain, GI upset, sweating, malaise, weight loss, tremor, insomnia, respiratory or urinary tract infections, headache, anxiety, agitation, depression, fatigue, wt loss. Caps/soln: Somnolence, sleep disorders, confusion, hallucination, hyperhidrosis, dehydration, tremor, hypertension, bradycardia, salivary hypersecretion, worsening of parkinson's disease. Patches: UTI, urinary incontinence, delirium, syncope, pyrexia. Skin reactions (discontinue if severe or persistent)
Pregnancy and breastfeeding
Pregnancy: No evidence of risk in humans. Lactation: Contraindicated or not recommended. Contraindications: Severe liver impairment. Precautions: Sick sinus syndrome, conduction defects, history or risk of GI ulceration, asthma, obstructive pulmonary disease, bladder outflow obstruction, renal or mild to moderate hepatic impairment. Monitor weight loss.
Advice
Take with meals. Oral dosing syringe may be used to withdraw oral solution from the container and then swallowed directly from the syringe. Do not crush or chew capsules, swallow whole. Apply patch firmly to clean, dry, hairless intact skin in a location, such as the back, that will not be rubbed by tight clothing and will not be affected by lotion, cream, or powder. Apply on a new site daily. Remove old patch before applying a new one. Avoid using same site within 14 days. Contact prescriber if there is acute headache, increased fatigue, agitation, confusion, dizziness, lack of sleep, depression, aggressiveness, increased salivation/sweating/tearing/urination, chest palpitations, increased…

Interactions with Rivastigmine
Depolarising neuromuscular blockers, cholinergic and anticholinergic agents. Drugs that cause bradycardia, torsade de pointes or QT prolongation.
