Antiparkinsonism Preps

Uses

Endocrine-based infertility. Prevention or suppression of puerperal lactation. Hyperprolactinaemia. Cyclical benign breast disease, pronounced mastalgia. Adjunct in acromegaly. Prolactinoma. Parkinsonism.

How to take it

Parkinsonism: Adults: Week 1:1 mg-1.25 mg at bed time. Week 2: 2 mg-2.5 mg at bed time. Week 3: 2.5 mg twice daily. Week 4: 2.5 mg 3 times daily increased further at 3 to 14 day intervals in increments of 2.5 mg to 10-40 mg daily in three divided doses. All doses should be taken during meals. Children: Not recommended. Endocrine based infertility: Adults: Initially 1 mg-1.25 mg at bed time. Titrate gradually at 2 to 3 days intervals to 7.5 mg daily in divided doses with meals; max 30 mg daily. Children: Not applicable. Prevention of lactation: Adults: Initially 2.5 mg on day of delivery, then 2.5 mg twice daily with meals for 14 days. Children: Not applicable. Suppression of lactation:…

Side effects

Nausea, postural hypotension, dizziness, headache, vomiting, constipation, nasal congestion, digital vasospasm, drowsiness, confusion, psychomotor excitation, hallucinations, dyskinesia, dry mouth, leg cramp. Retro-peritoneal fibrosis on long term therapy (discontinue if occurs). Rarely, hypertension, MI, seizures, stroke, psychiatric reactions.

Pregnancy and breastfeeding

Pregnancy: No evidence of risk in humans. Lactation: Contraindicated or not recommended. Contraindications: Toxaemia of pregnancy, puerperal and postpartum hypertension, uncontrolled hypertension, hypersensitivity to ergot alkaloids. Precautions: Hepatic impairment. Risk of sudden sleep onset. Monitor for impulse control disorders incl pathological gambling, hypersexuality, compulsive spending or eating. May induce ovulation; patients not wishing to conceive should use non hormonal contraception. Withdraw if conception occurs. Perform regular gynaecological assessment. Evaluate pituitary function. Advice

Take with meals. Take at the same time each day. Avoid alcohol during therapy. Do not take OTC sedatives and CNS depressants without consulting prescriber. If you miss a dose, take missed dose as soon as you remember, unless it is almost time for the next dose. Do not take double dose. Contact prescriber if there is unrelieved constipation, vomiting, acute headache, dizziness, chest pain, irregular heartbeat, nervousness, loss of memory, hallucinations, seizures, painful or difficult urination, skin rash, increased muscle spasm, rigidity or involuntary movements or if there is worsening of condition. Interactions with Bromocriptine

Alcohol, erythromycin, metoclopramide, drugs affecting BP, other ergot alkaloids, macrolides, octreotide, antipsychotics, domperidone, strong CYP3A4 inhibitors or substrates.
