About Rivaroxaban

Anticoagulant/Antiplatelet

Uses

Prevention of venous thromboembolism in patients undergoing elective hip or knee replacement surgery. Treatment of DVT and pulmonary embolism. Prophylaxis of stroke and systemic embolism in patients with non-valvular atrial fibrillation and with at least one of the following risk factors: CHF, hypertension, previous stroke or TIA, age = 75 yrs or diabetes mellitus. Prophylaxis of atherothrombotic events following an acute coronary syndrome with increased cardiac biomarkers (in combination with aspirin alone or aspirin and clopidogrel).

How to take it

Adults: Prevention of venous thromboembolism in patients undergoing elective hip or knee replacement surgery: 10 mg within 6-10 hr of surgery if haemostasis established then 10 mg once daily for 5 wk following hip surgery or 2 wk following knee surgery. Initial treatment of DVT and pulmonary embolism: Initially 15 mg twice daily for 21 days. Continued treatment of DVT and pulmonary embolism (following initial treatment): 20 mg once daily. Prophylaxis of stroke and systemic embolism in patients with non-valvular atrial fibrillation and with at least one of the following risk factors: CHF, hypertension, previous stroke or TIA, age = 75 yrs or diabetes mellitus: 20 mg once daily. Side effects

Anaemia, elevated GOT and transaminases, nausea, post-procedural haemorrhage. Dizziness, headache, hypotension, haematoma, epistaxis, haemoptysis, GI upset, pruritus, rash, pain in extremities, fever, oedema, fatigue, asthaenia, renal impairment, raised LFTs, contusion, wound secretions, serious skin reactions.

Pregnancy and breastfeeding

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Creatinine clearance <15 mL/min, clinically significant active bleeding, hepatic disease associated with coagulopathy and clinically relevant bleeding risk, hip fracture surgery. Precautions: Severe renal impairment. Cirrhotic patients with moderate hepatic impairment. GI ulceration, severe hypertension, bleeding disorders and other patients with increased bleeding risk. Spinal or epidural anaesthesia, lumbar puncture.

Advice

10 mg tab: Take with or without meals. 15-20 mg tab: Take with meals. Inform prescriber if any surgery is due before starting this medication.

Interactions with Rivaroxaban

Azole antifungals, protease inhibitors, NSAIDs, strong CYP3A4 inducers (e.g. rifampicin), other anticoagulants, antiplatelets, protease inhibitors, macrolides, SSRIs, SNRIs, dronedarone.
