About Repaglinide

Hypoglycaemic excluding Insulin

Uses

Non-insulin dependent (type II) diabetes uncontrolled by diet and exercise. In combination with metformin where control of hyperglycaemia is not satisfactory.

How to take it

Adults: Initially 0.5 mg, 15-30 min before each meal. Titrate at 1-2 wk intervals according to response. Maintenance 0.5-4 mg with meals. Max 4 mg as single dose before each main meal; max total daily dose 16 mg. If transferring from another oral hypoglycaemic, start therapy with 1 mg before each main meal. Elderly: Over 75 yr, not recommended. Children: Not recommended.

Side effects

Hypoglycaemia, visual disturbances, GI upset, rash, transient elevation in liver enzymes.

Pregnancy and breastfeeding

Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Type I diabetes, diabetic acidosis, severe renal/hepatic impairment. Precautions: Periodically monitor blood glucose and glycosylated haemoglobin.

Advice

Take 15-30 minutes before meals. If a meal is skipped or added, skip or add a dose. Contact prescriber if there is anxiety, confusion, dizziness, excessive sweating, headache, increased thirst, chest pain, muscle or back pain, increased urination or nausea. Follow dietary and lifestyle changes as prescribed. When therapy is started, changes in blood glucose level may cause vision changes which are temporary. Wear or carry identification card indicating diabetes. Keep candy or other source of sugar to treat mild hypoglycaemia. Laboratory testing of HbA1C level should be done every 3 to 6 months until blood glucose level is controlled.

Interactions with Repaglinide

Azole antifungals, erythromycin, mibefradil, rifampicin, phenytoin, gemfibrozil, clarithromycin, MAOIs, beta-blockers, ACE inhibitors, salicylates, NSAIDs, octreotide, alcohol, anabolic steroids, oral contraceptives, thiazide diuretics, corticosteroids, danazol, thyroid hormones, sympathomimetics. Barbiturates, phenobarbital, carbamazepine, trimethoprim, St John's wort, deferasirox, clopidogrel, other inhibitors or inducers of CYP3A4 or 2C8.
