About Ramipril
ACE Inhibitor
Uses
Mild to moderate hypertension. Symptomatic heart failure, prophylaxis after myocardial infarction in patients with clinical evidence of heart failure and when initial dose not tolerated. Prevention of cardiovascular events in patients with atherosclerotic cardiovascular disease or with diabetes mellitus and at least one additional risk factor for CV disease. Nephropathy.
How to take it
Adults: Hypertension: Initially 1.25 mg once daily, increasing to maintenance 2.5 mg-5 mg once daily. Max 10 mg once daily. Discontinue any diuretic 2-3 days before initiating therapy. Symptomatic heart failure (adjunct) (under close medical supervision): 1.25 mg once daily, increased to 10 mg daily if tolerated, taken in 1-2 divided doses, dose increased eventually at intervals of 1-2 wks. Prophylaxis after myocardial infarction in patients with clinical evidence of heart failure: Started at least 48 hrs after infarction. Side effects
Nausea, vomiting, dizziness, fatigue, headache, abdominal pain, diarrhoea, cough. Rarely, symptomatic hypotension, angioneurotic oedema, syncope, renal impairment, hepatic impairment, bronchitis, sinusitis, dyspnoea, GI upset, rash, muscle spasms, myalgia, chest pain, hyperkalaemia. Hypersensitive reactions. Reports of neutropenia/agranulocytosis.
Pregnancy and breastfeeding
Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: History of angioneurotic oedema. Aortic or mitral valve stenosis, renal artery stenosis, outflow obstruction, hypotension, haemodynamic instability. Extracorporeal treatments involving contact of blood with negatively charged membranes. Precautions: Congestive heart failure; initiate therapy in hospital. Hepatic impairment; initiate therapy under close medical supervision. Haemodialysis. Renal impairment; reduce dose and assess renal function prior to and during treatment. Advice
Take with or without meals. Take first dose at bedtime. Contact prescriber immediately if there is swelling of face/mouth/lips/tongue/throat, chest pain, irregular heartbeat, difficulty in breathing, constant cough, constant muscle pain, joint pain or back pain. Take in sitting position to prevent hypotension. Do not take potassium supplements or salt substitutes without consulting prescriber. Avoid alcohol during therapy. Change position slowly when climbing stairs and from lying or sitting to standing position.

Potassium sparing diuretics or potassium supplement, NSAIDs, other antihypertensives, sacubitril/valsartan, lithium, allopurinol, immunosuppressants, temsirolimus, everolimus, corticosteroids, procainamide, antidiabetic agents, alcohol, cytostatics, hyposensitising agents, trimethoprim, heparin, TCADs, anaesthetic, baclofen, high-flux dialysis memebranes, sympathomimetics.
