Indications

Major depression.

Dosage

ORAL: Adults: Initially 10 mg once a day, altered according to response to 5-20 mg once a day. Elderly: Initially 5 mg once a day, increased if required up to 20 mg once a day. Children: Under 18 years not recommended.

Adverse Effects

Abnormal dreams, constipation, diarrhoea, dizziness, nausea, pruritus, vomiting, flushing, night sweats, bleeding disorders, serotonin syndrome.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Positive evidence of risk: Use only when no safer alternative exists for a serious problem. Lactation: Contraindicated or not recommended. Precautions: Bleeding disorders, cirrhosis of the liver, elderly, history of mania/hypomania, history of seizures, unstable epilepsy. Severe renal or hepatic impairment. Monitor suicidal ideation. Other risk factors for hyponatraemia.

Interactions

Dapoxetine, dosulepin, linezolid, methylene blue, MAOIs, pimozide, tryptophan, urokinase, strong CYP2D6 inhibitors, broad CYP inducers (e.g., rifampicin, carbamazepine, phenytoin), 5HT1 agonists/triptans, tramadol, other serotonergic drugs, St John's wort, antipsychotics, dopamine antagonists, lithium, drugs that reduce seizure threshold (e.g., antidepressant, mefloquine), anticoagulants, NSAIDs and other drugs affecting platelet function, drugs that cause hyponatraemia.

Take with or without meals.

Metabolism: Hepatic. Bioavailability: 75%. Half-life elimination: Approx. 6 hrs. Time to peak: 7-11 hrs. Excretion: Urine (59%), faeces (26%).
