Indications

Erectile dysfunction.

Dosage

Adults: 10 mg taken 25-60 min prior to sexual activity. Increase to 20 mg or decrease to 5 mg as necessary. Max dose: 20 mg once daily. Children: Not recommended.

Adverse Effects

Headache, flushing, dizziness, nasal congestion, dyspepsia, nausea, serious cardiovascular events have been reported.

Risk Factor (Pregnancy/Lactation)

Contraindications: Pre-existing cardiac conditions where sexual activity inadvisable, non-arteritic anterior ischaemic optic neuropathy, severe hepatic impairment, ESRD requiring dialysis, hypotension, recent stroke or MI, hereditary degenerative retinal disorders. Precautions: Cardiovascular disease, left ventricular outflow obstruction, severe renal impairment, mild to moderate hepatic impairment, abnormal penile anatomy or predisposition to priapism including sickle cell anaemia, multiple myeloma or leukaemia. Active peptic ulcer, bleeding disorders, risk factors for QTc prolongation. Advise patients to report any changes in vision immediately. Elderly.

Interactions

Nitrates, other erectile dysfunction agents, indinavir, ritonavir, other CYP3A4 inhibitors, alpha blockers, nicorandil.

Advice to Patient

Take with or without meals. Take as directed, 1 hour before sexual activity. Contact prescriber immediately if there is acute head pain, sudden change in vision, hearing loss, seizures, chest pain, fever, chills, difficulty in breathing, rash, difficulty in remembering, swelling of genital areas or if erection remains for more than 4 hours or if there is severe penile pain. Do not take with grapefruit juice or meals containing high-fat. Be cautious while driving at night or in dim environments. Use condom to prevent sexually transmitted diseases. Have periodic follow-up appointments as advised by prescriber.

Onset of action: Approx. 60 mins. Absorption: Rapidly absorbed from the GI tract. Bioavailability: Approx. 15%. Metabolism: Hepatic. Half-life elimination: Terminal: Approx. 4-6 hrs. Time to peak, plasma: 0.5-2 hrs. Excretion: Mainly via faeces (approx. 91-95%). Urine (approx. 2-6%).
