Urofollitropin (FSH)

Indications

Anovulatory infertility unresponsive to clomifene. Development of multiple follicles as part of an Assisted Reproductive Technology (ART) cycle in ovulatory women who have previously received pituitary suppression.

Dosage

Anovulation: Adults: Initially 75 IU-150 IU daily by SC or IM inj. Monitor response. Controlled Ovarian hyperstimulation: Usually 150 IU- 225 IU daily by SC or IM inj. Children: Not applicable.

Administration

May be administered by IM or SC depending on indication. The recommended sites for SC injection are either side of the lower abdomen in alternating fashion. Injection into the thigh is not recommended. Reconstitution: Inject 1 mL of sterile saline for injection into the vial of urofollitropin. Do not shake, but gently swirl until the solution is clear. Stability: Store at 3°C-25°C (37°F-77°F). Protect from light. Use immediately after reconstitution. Discard unused material.

Adverse Effects

GI discomfort, breast tenderness, diarrhoea, constipation, nausea, vomiting, increased risk of infection, pain, headache, hot flush, muscle spasms, ovarian hyperstimulation syndrome, rash, pelvic pain, vaginal discharge, vaginal haemorrhage.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Ovarian cysts (not caused by PCOs), tumours of breast, tumours of hypothalamus, tumours of ovaries, tumours of pituitary, tumours of uterus, vaginal bleeding of unknown cause. Precautions: Monitor ovarian size and oestrogen response to avoid hyperstimulation. Risk factors for thromboembolic events.

Advice to Patient

Contact prescriber if there is severe abdominal pain, nausea, vomiting, diarrhoea, difficulty in breathing, unusual weight gain, less urination. Do not reuse needles, syringes or other materials. Keep all doctors and lab appointments.

Absorption: SC route has greater bioavailability than the IM route. Half-life elimination: Single dosing: SC: 31.8 hrs. IM: 37 hrs. Multiple dosing: SC: 20.6 hrs. IM: 15.2 hrs.
