Indications

Local or general fibrinolytic states, to prevent bleeding in (e.g. in prostatectomy, dental extraction in haemophilia). Hereditary angioedema, epistaxis, thrombolytic overdose. Menorrhagia.

Dosage

Adults: ORAL: Local fibrinolysis: 15-25 mg/kg body weight two-three times daily. Menorrhagia: Initiated when heavy bleeding started, 1-1.5 gm 3-4 times daily for 3-4 days. Hereditary angioedema: 1-1.5 gm 2-3 times daily. Epistaxis: 1 gm three times daily for 7 days. INJECTION: Local fibrinolysis: 0.5-1 gm by slow IV 3 times daily. General fibrinolysis: 1 gm (or 15 mg/kg) every 6-8 hr. Children: Injection/Infusion not recommended for children under 1 yr. Not recommended for reduction of blood loss during cardiac surgery. Inhibition of fibrinolysis, hereditary angioedema: ORAL: 1 mth-18 yr, 15-25 mg/kg (max 1.5 gm) 2-3 times daily. INJECTION: By IV injection over at least 10 minutes, 1-18 yr, 10 mg/kg (max 1 gm) 2-3 times daily. By continuous IV infusion, 1-18 yr, 45 mg/kg over 24 hr. Administration

IV injection or administer as IV continuous infusion, 25-50 mg/kg over 24 hours. Can be mixed with solutions for infusion such as NS or D5W. Stability: Store at 15°C-30°C (59°F-86°F).

Adverse Effects

Gastrointestinal disorders, nausea, vomiting, diarrhoea, hypotension, convulsions, rarely thromboembolic events, colour vision disturbances (discontinue); giddiness on rapid IV injection.

Risk Factor (Pregnancy/Lactation)

Pregnancy: No evidence of risk in humans. Lactation: Caution advised or effect undetermined. Contraindications: Thromboembolic disease. History of convulsions. Severe renal impairment. Precautions: Renal impairment, massive haematuria (avoid if risk of ureteric obstruction); perform regular eye examinations and liver function tests in long-term treatment of hereditary angioedema. Disseminated intravascular coagulation.

Advice to Patient

Take oral forms with or without meals. Swallow tablets whole, do not chew or break. Seek emergency care immediately if allergic reaction, especially dyspnoea, a feeling of throat tightness, and facial flushing are present and stop taking medication. Contact prescriber if change in vision or ocular discomfort occurs.

Metabolism: <5% drug is metabolized. Half-life elimination: 3 hrs. Excretion: Urine.
