Miotic, Antiglaucoma Preps · Beta-blocker
Indications
Ocular hypertension, chronic open angle glaucoma, secondary glaucoma.
Dosage
Adults: Initially 1 drop of 0.25% soln. twice daily changing to 0.50% soln. twice daily if required. Children: Not recommended.
Adverse Effects
Ocular irritation, transient blurred vision, bronchospasm. Systemic beta-blocker effects, exacerbation of Myasthaenia gravis, GI and CNS disturbances, allergic reactions, rash, headache, dizziness, cough, chest pain, alopecia.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Asthma, history of obstructive pulmonary disease. Sinus bradycardia, sick sinus syndrome, sino-atrial block, 2nd or 3rd degree AV block, uncompensated cardiac failure, cardiogenic shock, hypotension, severe Raynaud's disease, unstable angina. Soft contact lenses. Precautions: Metabolic acidosis, 1st degree AV block, history of severe cardiac disease, hyperthyroidism, diabetes, cardiac disorders, hypotension, severe peripheral circulatory disorders, corneal diseases, dry eye, surgery, myasthenia gravis, untreated phaeochromocytoma, renal or hepatic impairment. Advise patient to report any intercurrent ocular condition. Monitor cardiac function and treatment following filtration procedures. Withdraw gradually. Mild or moderate COPD.
Interactions
Floctafenine, amiodarone, MAOIs, verapamil, diltiazem, nifedipine, antihypertensives, adrenaline, oral Beta-blockers, digoxin, disopyramide, lidocaine, quinidine, clonidine, insulin, oral hypoglycaemics.
Advice to Patient
Wash hands properly before use. Remove contact lenses before use. Wait 15 minutes before reinserting. To instill medication into eyes, tilt head back, look upward, instill drops or apply thin layer inside lower eyelid. Close eye and roll eyeball in all directions. Do not blink for 30 seconds. Apply gentle pressure to inner corner of eye for 30 seconds. Contact prescriber if there is eye pain; eye damage; eye infection; redness, burning, watering, puffiness around eyes; changes in vision; loss of vision; no improvement or worsening of condition. Avoid touching tip of applicator with eye. Do not share medication with anyone else. Wait 5 minutes before instilling any other medication.

Onset: 30 mins. Duration: 12-24 hrs. Absorption: Significant systemic absorption when administered to the eye. Metabolism: Hepatic. Half-life elimination: 4 hrs. Time to peak: 1-2 hrs. Excretion: Urine (mainly).
