Ticarcillin and Clavulanic Acid
Broad-spectrum Penicillin · Beta-lactamase inhibitor/Broad-spectrum penicillin
Indications
Severe infections due to pseudomonas and proteus species in hospitalised patients, proven or suspected infections in patients with impaired or suppressed host defences.
Dosage
Adults: 3.2 gm 6-8 hourly by intermittent IV infusion. Max 3.2 gm 4 hourly. Children: Neonates, 80 mg/kg 12 hourly. Others, 80 mg/kg six to eight hourly. Child above 40 kg, adult dose. All by intermittent IV infusion. Septicaemia: Adults: >60 kg: 300 mg/kg/day ticarcillin given every 4 hr. <60 kg: 200-300 mg/kg/day ticarcillin given every 4-6 hr. Children: >3 mth & <60 kg: 50 mg/kg/day ticarcillin given every 4 hr. >3 mth & <60 kg: 3.2 gm every 4 hr. Bone infection: Adults: >60 kg: 300 mg/kg/day ticarcillin given every 4 hr. <60 kg: 200-300 mg/kg/day ticarcillin given every 4-6 hr. Children: >3 mth & <60 kg: 50 mg/kg/day ticarcillin given every 4-6 hr. >3 mth & <60 kg: 3.2 gm every 4-6 hr.
Administration
IV: Infuse over 30 min. Administer 1 hour apart from aminoglycosides. Give around-the-clock. Note: Reconstituted soln is stable for 6 hours at room temperature & 72 hours when refrigerated. IV infusion in NS is stable for 24 hours at room temperature, 7 days when refrigerated, or 30 days when frozen. Darkening of solution indicates loss of potency of clavulanate potassium.
Adverse Effects
Hypersensitivity, GI upset. Rarely, hepatitis, cholestatic jaundice, haematological and CNS effects, hypokalaemia, pseudomembranous colitis.
Risk Factor (Pregnancy/Lactation)
Pregnancy: No evidence of risk in humans. Lactation: Caution advised or effect undetermined. Contraindications: Hypersensitivity to penicillins, cephalosporins or other beta-Iactams. Precautions: Severe hepatic dysfunction. Moderate or severe renal impairment.

Advice to Patient
This medication is to be administered by injection or infusion only. Contact prescriber if there is burning/swelling/redness/pain at infusion site or if there is chest tightness, chest pain, difficulty in breathing or swallowing, persistent diarrhoea or abdominal pain, fever, chills, unhealed sores, rash, hives, swelling of lips or mouth, bloody stool or urine, muscle pain, mouth sores, white plaques in vagina or mouth, fatigue, purulent vaginal discharge. Take sufficient amount of fluids (2-3 litres per day) to maintain hydration, unless restricted by the prescriber.

TICARCILLIN: Absorption: Not absorbed from the GI tract. Metabolism: Partially metabolized. Half-life elimination: 70 mins. Prolonged in neonates and patients with renal/hepatic impairment. Severe renal impairment: 15 hrs. Time to peak, plasma: IM inj: 1 hr. Excretion: Urine (90%). CLAVULANIC ACID: Half-life elimination: 1 hr. Time to peak: 1.5 hrs. Excretion: Urine.
