Indications
Symptomatic treatment of urinary obstruction caused by benign prostatic hypertrophy. Mild to moderate hypertension.
Dosage
Adults: Urinary obstruction: Initially 1 mg once daily at bedtime. If necessary, increase at weekly intervals up to 10 mg daily. Hypertension: Initially 1 mg at bed time. Titrate by approx doubling dose at weekly intervals. Usual maintenance 2-10 mg once daily. Children: Not recommended.
Adverse Effects
Syncope with initial dose. Dizziness, postural hypotension, lassitude, peripheral oedema, asthenia, nausea, palpitations, somnolence, nasal congestion, blurred vision.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Risk cannot be ruled out. Lactation: Caution advised or effect undetermined. Precautions: History of syncope. Avoid rapid increase in dose.
Interactions
Beta-blockers, diuretics, ACE inhibitors, calcium channel blockers, sildenafil, tadalafil, vardenafil, NSAIDs, pressor agents (norepinephrine).
Advice to Patient
Take as advised by prescriber, at bedtime. If you miss a dose for several days, inform prescriber and do not restart with the previous dose. Contact prescriber if there is confusion, nervousness, lethargy, fatigue, rapid weight gain, swelling of feet/ankles/extremities, palpitations, fast heartbeat, difficulty in breathing, muscle weakness. Avoid standing for long time, exercising, alcohol and going outside in hot weather. Regularly check weight once a week at the same time of day in same clothes. Change position slowly when climbing stairs and from lying or sitting to standing position. Empty your bladder before taking medication. Have periodic follow-up appointments with prescriber.

Metabolism: Hepatic; minimal first-pass. Half-life elimination: Approx. 12 hrs. Time to peak, plasma: Approx. 1 hr; delayed approx. 40 mins with food. Excretion: Faeces (approx. 60%). Urine (approx. 40%).
