Tenecteplase

Indications
Acute myocardial infarction.
Dosage
Adults: Single bolus administered over 5 sec in individualized dosage based on patient’s weight, as follows: For patients weighing < 60 kg: 30 mg (6 mL), 60-69 kg: 35 mg (7 mL), 70-79 kg: 40 mg (8 mL), 80-89 kg: 45 mg (9 mL), = 90 kg: 50 mg (10 mL). Max: 50 mg total dose.
Administration
Administer drug as a single IV bolus over 5 sec. Reconstitution: To reconstitute, use supplied 10 mL syringe with dual cannula device. Withdraw 10 mL of supplied (preservative- free) SWFI into syringe and inject entire contents into vial containing tenecteplase dry powder, directing stream of diluent into powder. Gently swirl vial until contents are completely dissolved. Do not shake. If slight foaming occurs during reconstitution, allow drug to stand undisturbed for a few mins to allow large bubbles to dissipate. Then withdraw prescribed dose of tenecteplase from reconstituted drug in vial, using supplied syringe. Make sure that reconstituted preparation is a colourless to pale yellow transparent solution. Adverse Effects
Haemorrhage, reperfusion arrhythmias, hypotension, ecchymosis, epistaxis, GI or urogenital bleeding, raised temperature. Rarely, anaphylactoid reactions.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Positive evidence of risk: Use only when no safer alternative exists for a serious problem. Lactation: Caution advised or effect undetermined. Contraindications: Significant bleeding disorder, History of CNS damage. Haemorrhagic diathesis, severe uncontrolled hypertension, major surgery, biopsy of parenchyma organ, significant trauma within past 2 months including head trauma, prolonged cardiopulmonary resuscitation within past 2 weeks, acute pericarditis, subacute bacterial endocarditis, acute pancreatitis, severe hepatic dysfunction, active hepatitis, active peptic ulceration, arterial aneurysm, known arterial/venous malformation, neoplasm with increased risk of bleeding, history of stroke, TIA or dementia. Oral anticoagulants, drug affecting platelet function.
Advice to Patient
Contact prescriber if there is unusual bruising or bleeding, acute headache, joint pain, chest pain, vision changes or respiratory difficulty. To prevent bleeding, use soft toothbrush, floss with waxed floss, use electric razor, avoid scissors or sharp knives and harmful activities. Take strict bed rest to avoid injury. If bleeding occurs, report immediately and apply pressure to bleeding spot until bleeding stops completely.
