Indications

Treatment of essential hypertension. Treatment of cardiovascular events in patients with established atherosclerotic cardiovascular disease, or type 2 diabetes mellitus with target-organ damage.

Dosage

Adults: Hypertension: 20-40 mg once daily. Max: 80 mg once daily. Treatment of cardiovascular events in patients with established atherosclerotic cardiovascular disease, or type 2 diabetes mellitus with target-organ damage: 80 mg once daily. Children: Not recommended.

Adverse Effects

Upper respiratory tract infection, back pain, sinusitis, diarrhoea, pharyngitis. Influenza-like symptoms, pain, chest pain, lack of efficacy, fever, hyperkalaemia, syncope, hypotension, acute renal failure. Acute transient myopia, acute angle-closure glaucoma (discontinue).

Risk Factor (Pregnancy/Lactation)

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Cholestasis & biliary obstructive disorders, severe hepatic impairment. Refractory hypokalaemia, hypercalcaemia. Precautions: Impaired hepatic function. Bilateral renal artery stenosis or stenosis of the artery to single functioning kidney. Mild to moderate renal impairment. Patients who are vol-&/or Na-depleted by vigorous diuretic therapy, dietary salt restriction, diarrhoea or vomiting. Severe CHF; primary aldosteronism; aortic or mitral stenosis, obstructive hypertrophic cardiomyopathy. May impair glucose tolerance. Hereditary fructose intolerance; ischaemic cardiopathy or CV disease. Monitor serum K in patients at risk of hyperkalaemia. Renovascular hypertension. Elderly.

Interactions

Monitor digoxin & lithium levels during concomitant use. Other kaliuretic diuretics, laxatives. Corticosteroids, ACTH, amphotericin, carbenoxolone, penicillin-G potassium, salicylic acid & derivatives, K+ sparing diuretics, K+ suppl, other antihypertensives, heparin, cyclosporin, tacrolimus, trimethoprim, NSAIDs, baclofen, alcohol, barbiturates, antidepressants.

Advice to Patient

Take with or without meals. Contact prescriber if there is severe diarrhoea, swelling of ankles/face/hands/throat/lips, unusual weight gain, constant fatigue, dry cough, difficulty in breathing, chest pain, palpitations, CNS changes, abdominal disturbances, muscle pain, bone pain, cramps, tremors, urinary disturbances, or hearing or vision disturbances. Check blood pressure regularly at same time of day. Take enough amount of fluid to prevent dehydration during hot weather. Change position slowly when climbing stairs and from lying or sitting to standing position.
