Indications
Mild or moderate community acquired pneumonia, acute exacerbation of chronic bronchitis, acute sinusitis. Tonsillitis or pharyngitis caused by Group A beta streptococci when beta lactam antibiotics are inappropriate.
Dosage
Adults: 800 mg once daily for 5 days. Pneumonia: 800 mg once daily for 7-10 days. Children: Not recommended.
Adverse Effects
GI upset, raised liver enzymes, dizziness, headache, taste disturbance, vaginal moniliasis. Very rarely severe hepatitis, liver failure, angioneurotic oedema, anaphylaxis.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Risk cannot be ruled out. Lactation: Caution advised or effect undetermined. Contraindications: Personal or family history of QT prolongation, pseudomembranous colitis. Precautions: Hepatic or severe renal impairment, coronary heart disease, ventricular arrhythmia, hypokalaemia, hypomagnesaemia, bradycardia.
Interactions
Cisapride, ergot derivatives, pimozide, astemizole, terfenadine, simvastatin, atorvastatin, lovastatin, QT prolonging agents, CYP3A4 inhibitors, inducers or substrates, CYP2D6 substrates, digoxin, theophylline, warfarin.
Advice to Patient
Complete full course of therapy even if you feel better. Avoid quick changes in seeing near and far objects. For first 30 minutes after a dose, difficulty in focusing vision can occur. Contact prescriber if there are visual disturbances, fatigue, malaise, anorexia, nausea, abdominal tenderness, yellow skin, or changes in stool appearance, irregular heartbeat. Advise patient to stop treatment and to report if any signs of hepatic disease develop.
