Sultamicillin
Broad-spectrum Penicillin · Broad-spectrum penicillin/ Beta-lactamase inhibitor
Indications
Susceptible infections of skin and skin structure, intra-abdominal infection, gynaecological infections.
Dosage
IM or IV: Adults: Mild infections: 1.5-3 gm/day (sulbactam 0.5 gm + ampicillin 1 gm). Moderate infections: up to 6 gm/day (sulbactam 2 gm + ampicillin 4 gm). Severe infections: Up to 12 gm/day (sulbactam 4 gm + ampicillin 8 gm). The divided dosage administered every 6 hr or every 8 hr in less severe infections every 12 hr. Children: Neonates, infants & children, for most infections 150 mg/kg/day (50 mg/kg/day sulbactam + 100 mg/kg/day ampicillin). The divided dosage administered every 6 hr or every 8 hr (12 hr in neonates during the 1st wk of life), normally for 5 to14 day, until 48 hr after pyrexia and other abnormal signs have resolved.
Administration
Administer round the clock by slow injection over 10-15 min or by IV over 15-30 min.
Adverse Effects
Diarrhoea, rash, allergic reaction e.g. serum sickness, urticaria, bronchospasm, hypotension etc.
Risk Factor (Pregnancy/Lactation)
Pregnancy: No evidence of risk in humans. Lactation: Caution advised or effect undetermined. Contraindications: Hypersensitivity to penicillins. Precautions: Appearance of rash should be carefully evaluated.
Interactions
Disulfiram, probenecid, methotrexate, oral contraceptives.
Advice to Patient
Take with or without meals. Contact prescriber if there is burning/swelling/redness/pain at infusion site or if there is chest tightness, chest pain, difficulty in swallowing, acute anxiety, fever, unhealed sores, chills, purulent vaginal discharge, white plaques in mouth or vagina, fatigue. Take sufficient amount of fluids (2-3 litres per day) to maintain hydration, unless restricted by the prescriber.
Pharmacokinetics
Note: Sultamicillin is a mutual prodrug of ampicillin and sulbactam, a beta-lactamase inhibitor. Absorption: Readily absorbed. Metabolism: Hydrolyzed by enzymes in the intestinal wall, releasing ampicillin and sulbactam in equimolar proportions. Half-life elimination: Ampicillin: 0.74-1.33 hrs. Sulbactam: 0.65-1.2 hrs. Prolonged in patients with renal impairment. Time to peak, plasma: 1 hr. Excretion: Urine (primarily).
