Sulbactam and Amoxicillin

Broad-spectrum Penicillin · Beta-lactamase inhibitor/Broad-spectrum penicillin

Indications

Infections caused by susceptible organisms of upper and lower respiratory tract, genito-urinary tract, skin, soft tissue, bone and joints. Gonococcal infections.

Dosage

ORAL: Susp: Children: 2-12 yr, 1-2 tsp 8 hourly; Under 2 yr, 1 tsp 8 hourly. Tab: Adults: 1 tab 8 hourly. INJECTION: IM or IV. Adults: Mild infections: Sulbactam 0.5 gm/day. Moderate infections: Sulbactam 2 gm/day. Severe infections: Sulbactam 4 gm/day. The divided dosage administered every 8 hr. Children: 50 mg/kg/day sulbactam. The divided dosage administered every 8 hr. See literature.

Adverse Effects

Hypersensitivity reaction, GI upset. Rarely hepatitis, cholestatic jaundice, haematological and CNS effects, pseudomembranous colitis, increased INR.

Risk Factor (Pregnancy/Lactation)

Pregnancy: No evidence of risk in humans. Lactation: Caution advised or effect undetermined. Contraindications: Hypersensitivity to penicillins, cephalosporins or other beta-Iactams. Precautions: Renal impairment. Risk of crystalluria with high doses, all should be well hydrated. Monitor renal, hepatic and haematological function in neonates.

Interactions

Anticoagulants, allopurinol, probenecid. Disulfiram, methotrexate, oral contraceptives.

SULBACTAM: Half-life elimination: Children 1-12 yrs (normal renal function): Mean range: Approx. 0.7-0.9 hrs. Adults (normal renal function): 1-1.3 hrs. Excretion: Urine (approx. 75%-85%). AMOXICILLIN: Absorption: Oral: Immediate-release: Rapid with or without food. Extended-release: Rate of absorption is slower compared to immediate-release formulations; food reduces the rate but not extent of absorption. Half-life elimination: Adults: Immediate-release: 61.3 mins. Extended-release: 90 mins. Time to peak: Cap, oral susp: 1-2 hrs. Chewable tab: 1 hr. Extended-release: 3.1 hrs. Excretion: Urine.
