Indications
Community acquired pneumonia caused by chlamydia pneumoniae, H. influenzae, H. parainfluenzae, moraxella catarrhalis, mycoplasma pneumoniae, or streptococcus pneumoniae. Acute bacterial exacerbations of chronic bronchitis caused by C. pneumoniae, enterobacter cloacae, H. influenzae, H. parainfluenzae, klebsiella pneumoniae, M. catarrhalis, streptococcus aureus, streptococcus pneumoniae.
Dosage
Adults: 400 mg on day 1, followed by 200 mg once daily for 9 days. Typhoid & paratyphoid: 200-400 mg daily in two divided doses for 14 days. Children: Not recommended.
Adverse Effects
GI disturbances. Dizziness, headache, tremor, confusion, convulsions, sleep disorders, rashes, photosensitivity, joint pain. Judgement and dexterity may be impaired. Transient increase in serum creatinine. Haematological, hepatic and renal disturbances. Tachycardia. Rarely, vasculitis, pseudomembranous colitis, haemorrhagic bullae, Stevens-Johnson and Lyell syndrome, visual disturbances, transient hearing loss, tenosynovitis, impaired taste, smell.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Precautions: Epilepsy, history of CNS disorders. Severe renal impairment; reduce dose by half. Patients should be well hydrated. Patients with G-6-PD defects. Driving or operating machinery.
Interactions
Antiarrhythmic agents, astemizole, phenothiazines, tricyclic antidepressants, cisapride, theophylline, cimetidine, sucralfate, magnesium, aluminium or iron salts, anticoagulants, NSAIDs.
Advice to Patient
Take with or without meals. Do not take within 4 hours of taking antacids containing aluminium, calcium, or magnesium, laxatives that contain magnesium, or vitamins that contain iron or zinc. Take at the same time each day. Take missed dose as soon as possible. Do not take double dose. Inform prescriber and discontinue medication if there is tendon pain/inflammation, burning/redness/pain/swelling/irritation at infusion site, urticaria, itching, facial swelling, difficulty in breathing or swallowing, loss of consciousness, chest pain, palpitations, tingling. Contact prescriber if there is blistering, skin rash, skin burning, skin redness, severe constant diarrhoea or constipation, unusual fever or chills, foul-smelling vaginal discharge or vaginal itching, easy bleeding or bruising. Absorption: Well absorbed from the GI tract. Metabolism: Hepatic. Bioavailability: 90%. Half-life elimination: 20 hrs. Prolonged in patients with renal impairment. Time to peak, plasma: 3-6 hrs. Excretion: Urine and faeces (equal amounts).
