Indications

Urge incontinence, urinary frequency and urgency associated with overactive bladder syndrome.

Dosage

Adults: 5 mg once daily. Increase to 10 mg once daily if necessary. Children: Not recommended.

Adverse Effects

Dry mouth, GI upset, blurred vision. Reports of angioedema, anaphylaxis (discontinue), QT prolongation.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Urinary retention, severe GI conditions, toxic megacolon, myasthaenia gravis, narrow-angle glaucoma, haemodialysis, severe hepatic impairment, neurogenic detrusor overactivity. Precautions: Bladder outflow obstruction at risk of urinary retention, GI obstruction, risk of oesophagitis, reduced GI motility, severe renal or moderate hepatic impairment, autonomic neuropathy, hiatus hernia, oesophageal reflux.

Interactions

Inhibitors, inducers or substrates of CYP3A4, cholinergic agonists, anticholinergics, GI motility stimulants.

Advice to Patient

Take with or without meals. Do not chew or crush tablets, swallow whole with a full glass of water. Limit or avoid alcohol intake during therapy. Avoid exercising and other activities in hot weather to prevent heatstroke.

Pharmacokinetics

Metabolism: Extensively hepatic. Bioavailability: Approx. 90%. Half-life elimination: 45-68 hrs following chronic dosing; prolonged in severe renal (CrCl <30 mL/min) or moderate hepatic (Child-Pugh class B) impairment. Time to peak, plasma: 3-8 hrs. Excretion: Urine (69.2%). Faeces (22.5%).
