Sofosbuvir, Velpatasvir and Voxilaprevir

Indications
Chronic hepatitis C virus (HCV) infection in adults without cirrhosis or with compensated cirrhosis (Child-Pugh A) who have EITHER genotype 1,2,3,4,5, or 6 infection and have previously been treated with an HCV regimen containing an NS5A inhibitor OR genotype 1a or 3 infection and have previously been treated with an HCV regimen containing sofosbuvir without an NS5A inhibitor.
Dosage
Adults: 1 tab once daily orally for 12 wks. Children: Not recommended.
Adverse Effects
Headache, fatigue, diarrhoea, nausea. Rashes, sometimes with blister or angioedema like swelling, angioedema.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: HBV infection. Moderate or severe hepatic impairment. Precautions: Patient should be tested for evidence of current or prior HBV infection before initiating treatment.
Interactions
Etravirine, rifampin, amiodarone (avoid).
Advice to Patient
Take with meals. Swallow whole tab, do not chew or crush. Contact prescriber if there is nausea, tiredness, yellowing of skin or white part of eyes, easy bruising/bleeding, confusion, dark/black/bloody stool, loss of appetite, dark/brown urine, swelling of abdomen or pain on the upper right side of stomach, sleepiness, vomiting of blood or lightheadedness.

Time to peak: 3 hrs. Excretion: Urine (0.4%), faeces (94%). VOXILAPREVIR: Metabolism: Hepatic. Half-life elimination: 33 hrs. Time to peak, plasma: 4 hrs. Excretion: Faeces (94%; 40% as parent compound).
