Sofosbuvir and Velpatasvir

Antiviral, Systemic · HCV NS5A replication complex inhibitors/Nucleotide analogue

Indications

Chronic hepatitis C infection, with or without ribavirin.

Dosage

ORAL: Adults: 400/100 mg once a day for 12 wks (may extend to 24 wks in some circumstances). Children: Under 18 years not recommended.

Adverse Effects

Headache, fatigue, nausea, anaemia.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Precautions: Hepatitis B co-infection. Severe renal impairment. Child-Pugh class C cirrhosis, liver transplant recipients. Failure of previous NS5A inhibitor. Risk of HBV reactivation.

Interactions

Amiodarone, carbamazepine, modafinil, oxcarbazepine, P-glycoprotein/ABCB1 inducers, phenobarbital, primidone, rifabutin, rifapentine, tipranavir, lumacaftor, ranolazine, St John's wort, phenytoin, efavirenz, modafinil, antacids, H2 antagonist, PPIs, tenofovir, vit K antagonists. Substrates of P-gp, BCRP, OATP1B1 or OATP1B3 (e.g., digoxin, statins, dabigatran).

Advice to Patient

Take with or without meals.

SOFOSBUVIR: Metabolism: Hepatic. Half-life elimination: 0.5 hrs. Time to peak: 0.5-1 hr. Excretion: Urine (80%), faeces. (14%). VELPATASVIR: Metabolism: Hepatic. Half-life elimination: 15 hrs. Time to peak: 3 hrs. Excretion: Urine (0.4%), faeces (94%).
