Sofosbuvir

Indications
With ribavirin (with or without peginterferon alfa) for the treatment of chronic hepatitis C.
Dosage
Adults: 400 mg once daily with food. Continue for up to 24 wk. Children: <18 yr: Not recommended.
Adverse Effects
Blood dyscrasias, anorexia, weight loss, insomnia, depression, anxiety, agitation, headache, dizziness, migraine, attention disturbances, memory impairment, blurred vision, dyspnoea, cough, GI upset, dry mouth, raised bilirubin, rash, pruritus, alopecia, dry skin, arthralgia, myalgia, muscle spasm, pain, fatigue, irritability, chills, pyrexia, flu-like illness, nasopharyngitis, asthaenia.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Severe renal impairment (CrCl <30 mL/min). Precautions: Decompensated cirrhosis, patients awaiting liver transplantation. Co-infection with HBV. Severe renal impairment.
Interactions
Telaprevir, boceprevir, other direct-acting HCV antivirals. Modafinil, potent P-gp inducers (e.g. rifamycins, carbamazepine, phenytoin, phenobarbital), amiodarone, vit K antagonists.
Advice to Patient
Take with or without meals.

Note: The pharmacokinetic profile in paediatric patients =12 yrs of age is similar to adult patients. Metabolism: Hepatic. Half-life elimination: 0.4 hrs. Time to peak: Approx. 0.5-2 hrs. Excretion: Urine (80%). Faeces (14%).
