Sitagliptin
Hypoglycaemic excluding Insulin · Dipeptidyl peptidase-4 inhibitor
Indications
Monotherapy: For type II diabetes inadequately controlled by diet and exercise when metformin is contraindicated or not tolerated. Dual therapy: With metformin or a glitazone for type II diabetes inadequately controlled by diet, exercise and either metformin or a glitazone alone; or with a sulphonylureas in patients with an intolerance or contraindication to metformin, inadequately controlled by maximal tolerated doses of sulphonylureas alone. Triple therapy: With metformin and a sulphonylureas or a glitazone for type II diabetes inadequately controlled by dual therapy. With insulin: With or without metformin for type II diabetes inadequately controlled by diet, exercise and insulin alone.
Dosage
Adults: 100 mg once daily. Combination therapy, consider lower dose of sulfonylurea or insulin. Children: Not recommended.
Adverse Effects
Reports of pancreatitis and serious hypersensitivity reactions, including anaphylaxis and Stevens-Johnson syndrome. Hypoglycaemia, headache, bullous pemphigoid. Monotherapy: Osteoarthritis, pain in extremity, upper RTI. With metformin: GI upset. With a glitazone: Flatulence, peripheral oedema. With insulin: Influenza.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Diabetic ketoacidosis. Moderate to severe renal impairment. Precautions: Elderly.

Digoxin.
Advice to Patient
Take with or without meals. Contact prescriber if there is trouble in breathing, rash, swelling. If there is persistent severe abdominal pain, radiating to the back and accompanied by vomiting, stop taking medication and consult prescriber. Dosage adjustment is required in case of infection, trauma, surgery so consult prescriber for change in dosage. Monitor blood glucose level regularly and report changes to prescriber. Wear or carry identification card at all times indicating diabetes.
