Indications
Maintain or increase bone mass in undergoing long-term systemic corticosteroid therapy in post-menopausal women. Paget's disease. Treatment of post-menopausal osteoporosis to reduce risk of vertebral fractures. Treatment of established post-menopausal osteoporosis to reduce risk of hip fractures.
Dosage
All tab should be swallowed whole in an upright position, with a glass of plain water, at least 30 min before the first food or drink of the day or at least 2 hr from any food or drink at other times and at least 30 min before going to bed. Adults: Post menopausal women on systemic corticosteroid therapy: 5 mg tab daily. Paget's disease: 30 mg daily for two mth. Treatment of post-menopausal osteoporosis to reduce risk of vertebral/hip fractures: 5 mg once daily; efficacy for use longer than 1 yr has not been established. Alternatively, a dose of 35 mg once wkly on the same day of each wk has been demonstrated to be effective. Children: Not recommended.
Adverse Effects
Abdominal pain, GI upset, musculoskeletal pain, body pain, headache. Transient decrease in serum calcium and phosphate levels.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Hypocalcaemia, severe renal impairment. Precautions: History of oesophageal disorders that delay oesophageal transit or disturbance of bone and mineral metabolism. Maintain adequate dietary intake of calcium and vit D.
Interactions
Calcium, magnesium, iron or aluminium salts.
Advice to Patient
Take atleast 30 minutes before the first meal of the day or 30 minutes before going to bed and remain in an upright position for atleast another 1 hr. Take with a full glass of water. Do not chew or crush or suck tablet, swallow whole. Do not eat or drink or take any other medication within 2 hours. If you have to undergo a dental surgery, inform prescriber about it. Contact prescriber if there is acute stomach pain, unrelieved muscle pain/bone pain, leg cramps, chest pain, palpitations, heartburn, change in vision, increased dryness in eyes, swelling in extremities, flu-like symptoms, ringing in ears, pain and difficulty in swallowing. Avoid alcohol during therapy. Limit alcohol intake and smoking. Take dietary supplements of calcium or vitamin D as recommended. Regular mouth care is required during therapy.

Onset of action: May require weeks. Absorption: Rapid. Metabolism: None. Bioavailability: Poor, approx. 0.54% to 0.75%. Half-life elimination: Initial: 1.5 hours; Terminal: 480 to 561 hours. Time to peak; serum: 1 to 3 hours. Excretion: Urine (up to 85%); feces (as unabsorbed drug).
