Indications

With an aromatase inhibitor for locally advanced or metastatic hormone receptor positive, HER2-negative breast cancer in postmenopausal women.

Dosage

Adults: With an aromatase inhibitor, 600 mg once daily for 21 days followed by a 7 day rest period. Children: Under 18 years: Not recommended.

Adverse Effects

UTI, headache, GI upset, dysgeusia, insomnia, fatigue, syncope, oedema, asthenia, pyrexia, dyspnoea, epistaxis, eye disorders, back pain. Blood dyscrasias, anorexia, electrolyte disturbances, hepatotoxicity, altered LFTs, increased creatinine, wt loss, QT prolongation.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Peanut or soya allergy. Precautions: Moderate to severe hepatic impairment. Severe renal impairment. Risk of neutropenia, hepatotoxicity. Risk factors for QT prolongation; perform ECG at baseline and during treatment and monitor electrolytes. Critical visceral disease.

Interactions

Drugs that prolong QT interval, substrates of P-gp, BCRP, OATP1B1/1B3, OCT1, OCT2, MATE1 and BSEP (e.g., digoxin, metformin), caffeine. CYP3A4 inhibitors, inducers and substrates.

Metabolism: Extensively hepatic. Half life elimination: Terminal: ˜ 30 to 55 hours. Time to peak: 1 to 4 hours. Excretion: Feces 69%. Urine 23 %.
