Indications

Parkinson's disease, used alone or as adjunct to co-beneldopa or co-careldopa.

Dosage

Adults: Monotherapy: 1 mg daily. Adjunctive therapy: Initial: 0.5 mg once daily. Titrate: may increase to 1 mg once daily. Adjust dose of levodopa with concomitant use. Children: Not recommended.

Adverse Effects

Monotherapy: Headache, influenza, malaise, neck pain, allergic reactions, fever, angina, flatulence, leucopenia, arthritis, musculoskeletal pain, depression, vertigo, hallucinations, rhinitis, conjunctivitis, dermatitis, skin carcinoma, urinary urgency. Adjunctive therapy: Falls, neck pain, postural hypotension, GI upset, anorexia, arthralgia, carpal tunnel syndrome, wt loss, dyskinesia, dystonia, abnormal dreams, balance disorders, hallucinations, rash.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Caution advised or effect undetermined. Contraindications: Moderate to severe hepatic impairment. Precautions: Mild hepatic impairment, avoid abrupt withdrawal. Monitor for impulse control disorders incl pathological gambling, hypersexuality, compulsive spending or eating. Risk of sudden sleep onset; warn patient.

Interactions

Other MAOIs, St. John's Wort, pethidine, fluoxetine, fluvoxamine, dextromethorphan, sympathomimetic, antidepressants, CYP1A2 inhibitors.

Advice to Patient

Take with or without meals. Discontinue medication and inform prescriber immediately if there is blurred vision, chest pain, difficulty in thinking, stupor or coma, seizures, severe headache, stiff neck, nausea, vomiting, palpitations, or evidence of stroke. Contact prescriber if hallucinations occur. Avoid foods rich in tyramine content such as aged cheese. Change position slowly from sitting or lying to standing.

Half-life elimination: Approx. 3 hrs (no correlation with biologic effect due to irreversible inhibition). Time to peak, plasma: Approx. 1 hr. Excretion: Urine (62%). Faeces (7%).
