Piroxicam-beta-cyclodextrin
Anti-rheumatic, Systemic · Oxicam
Indications
Acute and chronic painful disorders: Musculo-skeletal pain (rheumatic pain, osteoarthritis, ankylosing spondylitis), post-operative pain, traumatic pain, dysmenorrhoea, headache including migraine, toothache.
Dosage
Adults: 20 mg daily as a single dose, can be increased to 40 mg daily. Elderly, Patients can be given 10 mg (half tab). Children: Not recommended.
Adverse Effects
Gl disturbances, skin reactions, oedema, CNS effects, malaise, tinnitus.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Positive evidence of risk: Use only when no safer alternative exists for a serious problem. Lactation: Caution advised or effect undetermined. Contraindications: Aspirin/anti-inflammatory induced allergy. Active peptic ulcer, history of recurrent ulceration. Precautions: Impaired renal or hepatic function. Monitor on long term therapy. Heart failure; elderly. Phenylketonuria.
Interactions
Anticoagulants, other NSAIDs, hypoglycaemics, lithium.
Advice to Patient
Take immediately after meals. Contact prescriber if there is severe diarrhoea, skin irritation, nausea, vomiting, dizziness, stomach upset, convulsions.
Pharmacokinetics
Absorption: Dissociates in the GI tract to piroxicam and betadex. Absorption is more rapid than that of unmodified piroxicam. Metabolism: Hepatic. Half-life elimination: 50 hrs. Time to peak, plasma: Oral: 30-60 mins. Excretion: Urine (mainly). Faeces.
