Indications
Essential hypertension and heart failure.
Dosage
Adults: Perindopril (as terbutylamine): Hypertension: 4 mg once in the morning, may be increased to 8 mg once in the morning, if necessary, after one month of treatment. Maximum 8 mg daily. Elderly: Initially 2 mg once in the morning for one month, then adjusted according to response. Maximum 8 mg daily. Hypertension, if used in addition to diuretic, or in cardiac decompensation or volume depletion: 2 mg once in the morning, may be increased according to response, if necessary, after one month of treatment. Maximum 8 mg daily. Heart Failure (adjunct): Initially 2 mg once daily in the morning for 2 wk, then increased if tolerated to 4 mg once daily. Prophylaxis of cardiac events following myocardial infarction or revascularization in stable coronary artery disease: Initially 4 mg once daily in the morning for 2 wks, then increased if tolerated to 8 mg once daily. Adverse Effects
Localised skin rashes, or flushing, angioneurotic oedema, nausea, abdominal pain, fatigue, asthaenia, headache, mild cough. Decreases in haemoglobin, red cells and platelets. Reversible increase in blood urea and creatinine. Proteinuria, pruritus, hypotension, dysgeusia, GI upset, muscle cramps, paraesthesia, dizziness, visual disturbance, tinnitus, dyspnoea. Hyperkalaemia, rarely, blood dyscrasias.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: History of angioedema associated with previous ACE inhibitor therapy, hereditary/idiopathic angioedema. Precautions: Renal impairment, surgery/anaesthesia, Risk factors for hypotension. Aortic and mitral valve stenosis, hypertrophic cardiomyopathy, unstable angina during first month of treatment, haemodialysis. LDL-apheresis with dextran sulfate, desensitisation, black care, hyperkalaemia. Monitor serum electrolytes.
Interactions
K+ sparing diuretics or K+ supplements, lithium, antidepressants, other antihypertensives, NSAIDs, high dose aspirin, antidiabetic agents, antipsychotics, TCADs, vasodilators, anaesthetics, sympathomimetics, hyposensitising agents, immunosuppressants, heparin, sodium aurothiomalate (gold), estramustine, baclofen, trimethoprim, everolimus, racecadotril, high-flux dialysis memebranes, alcohol.
Advice to Patient
Take before meals. Take first dose at bedtime. Contact prescriber if there is fever, sore throat, chest pain, difficulty in breathing, constant cough, muscle pain, joint pain, rash or ringing in ears. Do not take potassium supplements or salt substitutes without consulting prescriber. Change position slowly.
