Plain, Medium & Narrow-spectrum Penicillin · Penicillinase-sensitive penicillin
Indications
Respiratory tract infections in children, tonsillitis, otitis media, erysipelas, cellulitis. Prophylaxis of pneumococcal infection and of streptococcal infection following rheumatic fever.
Dosage
Lower Respiratory Infection: Adults & Children: > 12 yr: Pneumococcal: 250-500 mg every 6 hr until afebrile for 2 days. Skin Infection: Adults & Children: >12 yr: 250-500 mg every 6-8 hr. Upper Respiratory Infection: Adults & Children: >12 yr: Streptococcal: 125-500 mg every 6 hr for 10 days. Fusospirochetosis: (Oropharynx) Adults & Children: >12 yr: 250-500 mg every 6-8 hr. Pneumococcal: Adults & Children: >12 yr: 250-500 mg every 6 hr until afebrile for 2 days.
Adverse Effects
Hypersensitivity reactions, including rash, urticaria, joint pains, angioedema, anaphylaxis, serum sickness-like reactions. Rarely, haemolytic anaemia, interstitial nephritis, blood dyscrasias, seizures, diarrhoea and antibiotic-associated colitis.
Risk Factor (Pregnancy/Lactation)
Pregnancy: No evidence of risk in humans. Lactation: Caution advised or effect undetermined. Precautions: History of allergy. Renal impairment. Hypersensitivity to penicillins, cephalosporins or other beta-lactams.
Interactions
Methotrexate, probenecid.
Advice to Patient
Take 1 hour before or 2 hours after meals. Take at regular intervals, as scheduled. Take missed dose as soon as you remember unless it is time for the next dose. Do not take double dose. Inform prescriber if diarrhoea occurs, even 2 months or more after penicillin therapy ends. Contact prescriber if you experience unhealed sores, chills, fever, white plaques in vagina or mouth, fatigue, purulent vaginal discharge, itching, hives, rash, swelling of tongue/mouth/lips/throat. Complete full course of therapy even if you feel better. Take sufficient amount of fluids (2-3 litres per day) to maintain hydration, unless restricted by the prescriber. Patient who uses oral contraceptives should use an additional form of contraception during therapy.
