Indications
Prevention and treatment of secondary hyperparathyroidism in dialysis patients with end renal disease.
Dosage
ORAL: Adults: Secondary hyperparathyroidism associated with chronic renal failure (stage 5 CKD): Initial dose (mcg) = iPTH level (pg/mL) divided by 80. Administered 3 times/wk, not frequent than every other day. Initial dose only after baseline serum calcium adjusted to < 9.5 mg/dL. Titration dose (mcg) = most recent iPTH level (pg/mL) divided by 80. Modest titration dose (mcg) = most recent iPTH level (pg/mL) divided by 100. Modest initial and dose titration is required where monitoring of iPTH, calcium and phosphorous is available less frequently than once per wk. Dosage adjustment for hypercalcaemia or elevated Ca x P: Decrease calculated dose by 2-4 mcg. If further adjustment required, dose interrupted or reduced until parameters are normalized. Administration
Administer as an IV bolus. Stability: Store at 25°C; excursions are permitted to 15°C-30°C. After initial vial use the contents of multi-dose vial remain stable up to 7 days when stored at controlled room temperature. Discard unused portion.
Adverse Effects
Hypercalcaemia, hypocalcaemia, parathyroid disorder, rash, pruritus, hyperphosphataemia, taste perversion, headache, GI upset, dizziness, acne, decreased appetite, breast tenderness.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Positive evidence of risk: Use only when no safer alternative exists for a serious problem. Lactation: Caution advised or effect undetermined. Contraindications: Hypercalcaemia, vit. D toxicity.
Interactions
Cardiac glycosides, ketoconazole, phosphate or vit. D-related agents, aluminium, calcium and magnesium containing agents, thiazide diuretics.
Advice to Patient
Follow a diet high in calcium and low in phosphorus contents. Contact prescriber if there is abdominal pain, stomach disturbances, blood in stool, constant fever, palpitations, chest pain, signs of respiratory infection, flu. Inform prescriber immediately if you take digoxin and its toxicity occurs. Avoid taking other phosphate or vitamin D related compounds during therapy. Phosphate binders might be recommended by the prescriber to control serum phosphorus level. Keep your feet in elevated position when sitting to prevent them from swelling.
