Indications
In combination with 5-FU and folinic acid for the treatment of metastatic colorectal cancer and stage III colon cancer after complete resection of primary tumour.
Dosage
Adults: Metastatic colorectal cancer: 85 mg/m² every 2 wk by IV infusion over 2-6 hr prior to administration of 5-FU. Stage III colon cancer: 85 mg/m² every 2 wk for 12 cycles by IV infusion over 2-6 hr prior to administration of 5-FU. Children: Not recommended.
Administration
For administration by IV inf, the liquid concentrated injection must be diluted in 250 to 500 mL of D5W. Chloride containing solutions must not be used for dilution. Compatibility: Incompatible with alkaline solutions (e.g. fluorouracil) and chloride-containing solutions. Flush infusion line with D5W prior to and following, administration of concomitant medications via same IV line. Stability: Store intact vials at room temperature of 25°C (77°F) and under normal lighting; excursions permitted to 15°C-30°C (59°F-86°F). Reconstituted solutions may be stored up to 24 hours at room under refrigeration at 2°C-8°C (36°F-46°F). Diluted solution is stable up to 6 hours at room temperature of 20°C-25°C (68°F-77°F) or up to 24 hours under refrigeration at 2°C-8°C (36°F-46°F).
Adverse Effects
Peripheral sensory neuropathy, pharyngolaryngeal dysaesthesia, anaemia, neutropenia, thrombocytopenia, nausea, diarrhoea, mucositis/stomatitis, alopecia. Raised liver enzymes, allergic reactions, ototoxicity, renal function disturbance, transient loss of visual acuity. Blood dyscrasias, GI upset, mucositis/stomatitis, anorexia, infections, hyperglycaemia, hypokalaemia, hyponatraemia, dehydration, depression, insomnia, dysgeusia, sensory disturbance, headache, dizziness, meningism, visual disturbance, haemorrhage, flushing, VTE, dyspnoea, coughing, hiccups, skin and inj site reactions, hyperhidrosis, alopecia, urinary disorders, pain, fatigue, asthenia, fever, wt change. Reports of hepatic vascular disorders.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Positive evidence of risk: Use only when no safer alternative exists for a serious problem. Lactation: Contraindicated or not recommended. Contraindications: Severe renal impairment, neutropenia (<2x109/litre), thrombocytopenia (<100x109/litre), peripheral sensory neuropathy. Precautions: Moderate renal impairment; monitor renal function. History of sensitivity to platinum compounds; monitor for allergic reactions, discontinue immediately if anaphylactoid reactions develop and do not re-challenge. Grade 4 diarrhoea. Warn patient of possibility of persistent sensory neuropathy following treatment. Perform a neurological examination before each dose; discontinue or reduce dose if paraesthesia or dysaesthesia are present. Perform full blood count with white cell differential before each dose; delay or reduce dose if Grade 3-4 neutropenia or thrombocytopenia has occurred.…
Advice to Patient
This medication is administered by infusion. Report immediately redness/pain/burning/swelling/itching at infusion site or if there is swelling of lips or mouth, rash, hives, chest tightness, back pain, difficulty in breathing or swallowing. Contact prescriber if there is chest pain, palpitations, unusual bruising or bleeding, numbness/tingling/pain/loss of sensation of extremities, swelling or pain in legs, unusual fatigue, changes in hearing, muscle cramps, twitching. Take sufficient amount of fluids (2-3 litres per day) to maintain hydration, unless restricted by the prescriber. Take small, frequent meals to maintain good nutritional status. Avoid highly crowded places to prevent infection. Do not get vaccinated without consulting prescriber. Using soft toothbrush, cotton swabs, waxed dental floss, regular mouth care, frequent rinses may help to prevent mouth sores.

Adults: Oxaliplatin ultrafilterable platinum: Distribution: Alpha phase: 0.4 hours; Beta phase: 16.8 hours; Terminal: 391 hours. Excretion: Urine (approx.54%); feces (approx.2%).
