Olmesartan Medoxomil and Amlodipine

Angiotensin-II Antagonist · Angiotensin II antagonist/Calcium antagonist

Indications

Hypertension, not adequately controlled by amlodipine or olmesartan.

Dosage

Adults: 1 tab daily using the strength as clinically appropriate. Children: Not recommended.

Adverse Effects

Dizziness, headache, oedema, fatigue, hyperkalaemia.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Hepatic or severe renal impairment, cardiogenic shock, acute MI, unstable angina, biliary obstruction. Precautions: Salt or volume depletion, severe congestive heart failure, renal disease, renal artery stenosis, aortic or mitral valve stenosis, obstructive hypertrophic cardiomyopathy, primary aldosteronism. Mild to moderate renal impairment; monitor serum potassium in patients at risk of hyperkalaemia. Risk of sprue-like enteropathy (discontinue).

Interactions

Lithium, K-supplements, K+-sparing diuretics, salt substitutes, NSAIDs, CYP3A4 inducers and inhibitors, sildenafil, other antihypertensives.

Advice to Patient

Take with or without meals. Do not chew. Swallow whole tablet with a glass of water. Contact prescriber immediately if there is allergic reaction (swelling of face/lips/tongue, rash, itching, hives), severe light-headedness, dizziness. Avoid grapefruit and grapefruit juice during treatment. Do not discontinue medication without consulting prescriber.

OLMESARTAN: Metabolism: GI tract. Bioavailability: 26%. Half-life elimination: Terminal: 13 hrs. Time to peak: 1-2 hrs. Excretion: Faeces (50%-65%). Urine (35%-50%). AMLODIPINE: Onset of action: Antihypertensive effect: Significant reductions in blood pressure at 24-48 hrs after first dose. Duration: Antihypertensive effect: At least 24 hrs. Discontinuation after 6-7 wks of treatment: Duration extends to at least 72 hrs. Absorption: Well absorbed. Metabolism: Hepatic. Bioavailability: 64%-90%. Half-life elimination: Terminal (biphasic): 30-50 hrs. Time to peak, plasma: 6-12 hrs. Excretion: Urine.
