Indications

Treatment of essential hypertension.

Dosage

Adults: Start with 20 mg once daily, may be increased to a max 40 mg per day or hydrochlorothiazide therapy may be added, if necessary. Effect may be seen within 2 wk and max by 8 wk after initiating therapy. Children: Not recommended.

Adverse Effects

Dizziness, pain, hypertriglyceridaemia.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Biliary obstruction, primary aldosteronism, severe hepatic or severe renal impairment. Precautions: Intra vascular volume depletion, aortic or mitral valve stenosis, obstructive hypertrophic cardiomyopathy, primary aldosteronism, hepatic impairment, lithium. Excessive BP decrease in patients with ischaemic heart disease or ischaemic cerebrovascular disease could result in MI or stroke. Ensure adequate hydration. Mild to moderate renal impairment, severe CHF, renal artery stenosis, other conditions stimulating the renin-angiotensin-aldosterone system; monitor serum potassium and creatinine. Risk of sprue-like enteropathy (discontinue). Elderly.

Interactions

K+-sparing diuretics, K+ supplements, NSAIDs, lithium, salt substitutes, heparin, cyclosporin, tacrolimus, antacids, trimethoprim, other antihypertensives.

Advice to Patient

Take with or without meals. Contact prescriber if there is prolonged diarrhoea, nausea or vomiting, joint pain, back pain, chest pain, palpitations, unrelieved headache, flu-like symptoms or upper respiratory infection. Avoid exercising in hot weather, avoid excessive intake of alcohol. Regularly check blood glucose level and inform prescriber about changes.

Metabolism: GI tract. Bioavailability: 26%. Half-life elimination: Terminal: 13 hrs. Time to peak: 1-2 hrs. Excretion: Faeces (50%-65%). Urine (35%-50%).
