Indications

For the inhibition of intra-operative miosis.

Dosage

Adults & Children: 1 drop 3 times daily starting on the day before surgery and continuing for 2 wk after surgery.

Adverse Effects

Transient stinging. Punctate keratitis, blurred vision, ocular irritation. Corneal epithelial breakdown incl perforation (discontinue).

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Caution advised or effect undetermined. Contraindications: Herpes simplex keratitis. Aspirin/anti-inflammatory induced allergy or asthma. Soft contact lenses. Precautions: Acute eye infections may be masked by the use of topical NSAID. Avoid sunlight. Complicated ocular surgery, corneal denervation, corneal epithelial defects, diabetes, ocular surface diseases, rheumatoid arthritis, repeated surgery within short time period, bleeding tendencies.

Interactions

Topical steroids, prostaglandin analogues, anticoagulants, antiplatelets.

Advice to Patient

Wash hands properly before use. Remove contact lenses before use. Wait 15 minutes before reinserting. To instill medication into eyes, tilt head back, look upward, instill drops or apply thin layer inside lower eyelid. Close eye and roll eyeball in all directions. Do not blink for 30 seconds. Contact prescriber if there is eye pain, changes in vision, loss of vision, no improvement or worsening of condition. Apply gentle pressure to inner corner of eye for 30 seconds. Avoid touching tip of applicator with eye. Do not share medication with anyone else. Wait 5 minutes before instilling any other medication.

Pharmacokinetics

Absorption: Penetrates the cornea. Minimal systemic absorption. Metabolism: Converted by ocular tissue hydrolases to active form amfenac.
