Nateglinide

Hypoglycaemic excluding Insulin · Prandial glucose regulator

Indications

With metformin in type II diabetes inadequately controlled by a maximally tolerated dose of metformin alone.

Dosage

Adults: Initially 60 mg within 1 to 30 min before meals (usually breakfast, lunch and dinner). Adjust dose according to glycosylated haemoglobin levels. Usual maintenance, 120 mg before meals; max 180 mg three times daily. Children: Not recommended.

Adverse Effects

Hypoglycaemia symptoms, GI upset. Rarely, hypersensitivity, raised liver enzymes.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Type I diabetes, diabetic ketoacidosis, severe liver disease. Precautions: Moderate hepatic impairment. Risk of hypoglycaemia especially in elderly, malnourished or those with adrenal or pituitary insufficiency.

Interactions

ACE inhibitors, diuretics, CYP2C9 inhibitors, corticosteroids, beta 2 agonists.

Advice to Patient

Take 30 minutes before meals. If a meal is skipped or added, skip or add a dose too. Contact prescriber if there is persistent tachycardia, sweaty palms, tingling of lips and tongue, seizures, change in sensations, nausea or vomiting, dizziness, weight gain, mild muscle pain or flu-like symptoms. Follow prescribed dietary and lifestyle recommendations. Avoid alcohol. Do not exercise excessively.
