Naproxen and Esomeprazole

Anti-rheumatic, Systemic · Propionic acid/Proton pump inhibitor

Indications

Osteoarthritis, rheumatoid arthritis and ankylosing spondylitis in patients at risk of NSAID-associated gastric and/or duodenal ulcers and where lower doses of naproxen or other NSAIDs are not sufficient.

Dosage

Adults: 1 tab twice daily, 30 min before food. Children: Not recommended.

Adverse Effects

Dizziness, headache, dysgeusia, hypertension, GI upset, arthralgia, oedema. GI bleeding, ulceration or perforation, rash. Serious skin reactions (discontinue) and hepatic reactions.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Severe heart failure, renal (CrCl <30 mL/min) or hepatic impairment. Active peptic ulcer, GI or cerebrovascular bleeding or other bleeding disorders. Aspirin/NSAID allergy. Precautions: Elderly. Mild to moderate renal, hepatic impairment or heart failure, cirrhosis, salt and/or volume depletion; monitor renal function. Porphyria, SLE, mixed connective tissue disease, ulcerative colitis, Crohn's disease, asthma, hypertension, ischaemic heart disease, peripheral arterial and/or cerebrovascular disease, CV risk factors (e.g. hyperlipidaemia, diabetes, smoking), coagulation disorders. History of GI disorders; warn patients to report any unusual abdominal symptoms. Perform ophthalmic examination if any visual changes occur.

Interactions

Antiretrovirals, corticosteroids, anticoagulants, antiplatelets, SSRls, cyclosporin, tacrolimus, diuretics, ACE inhibitors, cardiac glycosides, lithium, methotrexate, hydantoins, sulfonamides, sulfonylureas, clopidogrel, beta-blockers, ketoconazole, itraconazole, cholestyramine, probenecid, quinolones.

Advice to Patient

Take atleast 30 minutes before meals. Do not chew or crush or break tablets, swallow whole.
